Objective: To study undiagnosed diseases in older people with and without dementia. Design: Cross-sectional population-based study in Lieto, southwestern Finland. Participants: All the inhabitants aged 64 and more in Lieto. Participation rate was 82% (n = 1,260). Measurements: Dementia and its subtypes were diagnosed according to prevailing criteria. Medical conditions were assessed in clinical examinations and from medical records. Results: 112 patients with dementia were found; 66% of them had at least 1 undiagnosed disease compared to 48% of the non-demented group (p = 0.041). The demented subjects had more undiagnosed hypercholesterolaemia (p = 0.039) and undiagnosed hypothyroidism (p = 0.032) than the controls. Conclusion: Undiagnosing is more common among patients with dementia. Screening strategies should be developed further to find these patients.

Objective: To explore ethnoracial and gender specific mortality associated with dementia hospitalizations from 1997 to 2008, using a nationally representative database.Design: Cross-sectional.Participants: 354,949,163 from the Nationwide Inpatient Sample (NIS) database using appropriate ICD-9 and procedure codes. Measurements:Descriptive, univariate and multivariable analysis (Linear, Cox) adjusting for comorbidity, hospital factors and socio-demographics were used.Results: Mortality was higher for dementia hospitalizations for all age groups (35-64 years and ≥ 65 years) vs. non-dementia hospitalizations (2.7% vs. 1.5% and 5.5% vs. 4.5%). For individuals aged 35-64 years, dementia hospitalizations were more common among males vs. females (53.8% vs. 46.2%). Crude in-hospital mortality was higher among Whites and males for all age groups and overall mortality declined from 1999 to 2008. Adjusted relative risk of mortality was higher among men as compared to women of all age groups (RR 2.87, 95% CI 2.82-2.92) and also higher among Hispanics and African Americans as compared to Whites (African Americans: RR 2.35, 95% CI 2.21-2.51; Hispanics: RR 2.15, 95% CI 2.06-2.23). Conclusion:African Americans, Hispanics and men bear a disproportionate burden from dementia in the hospital setting. Interventions to improve care outcomes in these populations are important.

ResumenAntecedentes: estudios previos han encontrado que el consumo de fl avonoides se asocia a menor riesgo de deterioro cognitivo. Sin embargo, el efecto del consumo habitual de chocolate sobre el estado cognitivo no está bien esclarecido. Objetivo: examinar si el consumo habitual de chocolate se asocia con un mejor estado cognitivo en adultos mayores españoles. Metodología: estudio transversal, utilizando datos del estudio ENRICA-Senior. El consumo habitual de chocolate fue evaluado con una historia dietética informatizada. Los participantes fueron divididos en las siguientes categorías, de acuerdo a su consumo de chocolate: sin consumo, < 10 g/d y ≥ 10 g/d. El estado cognitivo fue determinado mediante la versión del test Mini-Mental State Examination (MMSE) para España. Los puntos de corte para defi nir deterioro cognitivo leve (DCL) fueron: ≤ 25, ≤ 24 y ≤ 23. Se utilizaron modelos de regresión lineal y logística ajustados por los principales factores confusores para calcular coefi cientes beta y odds ratios (OR) de asociación. Resultados: en comparación con el grupo de personas que no reportaron consumir chocolate, aquellos con un consumo habitual de ≥ 10 g/d obtuvieron una mayor puntuación en el MMSE (coefi ciente beta e intervalo de confi anza al 95% [IC 95%] del modelo ajustado: 0,26 [0,02-0,50]; p de tendencia lineal = 0,05). Cuando examinamos la asociación solo con chocolate negro, los resultados también fueron signifi cativos (coefi ciente beta: 0,48 [IC 95%: 0,18-0,78]; p de tendencia lineal < 0,001). El consumo de chocolate total no se asoció con DCL en ninguna de las defi niciones propuestas. Sin embargo, el consumo de chocolate negro se asoció con menor probabilidad de DCL (OR e IC 95% para MMSE Conclusión: estos resultados sugieren que puede existir una asociación entre consumo de chocolate y mejor función cognitiva en personas mayores. Objective: To assess the infl uence of habitual chocolate consumption over the MMSE in Spanish older adults. Methodology: Cross-sectional study, using data of the follow-up of the Seniors-Study on Nutrition and Cardiovascular Risk in Spain (ENRICA) cohort. Habitual chocolate consumption in the last year was assessed with a computerized dietary history; differences between dark chocolate and milk chocolate were recorded. Chocolate intake was classifi ed into the following categories: no consumption, < 10 g/day, and ≥ 10 g/day. Validated MMSE scores for Spain were obtained during an interview and different cutoff points were used to defi ne ≤ 25, ≤ 24 and ≤ 23. Linear and logistic regression models were used to calculate adjusted beta coeffi cients and odds ratios (OR).

Co-morbidity associated with dementia

Abstract: There are distinct differences in comorbid conditions among dementia subtypes. Our research does not support previous studies that suggest a circulatory component to the development of Alzheimer's disease.
