Coagulation Factor XIII Reduces Postoperative Bleeding After Coronary Surgery with Extracorporeal Circulation

Abstract: Factor XIII administration reduces postoperative blood loss and the extent of blood transfusion after coronary surgery, however administration is only helpful if plasma levels are below the normal value. Measurement of plasma levels is recommended before Factor XIII substitution.

“…Patients treated with placebo had the most bleeding and those treated with 2500 units had the least with the 1250 unit group being intermediate, but these differences were not statistically significant. Patients who had post-administration levels of factor XIII <70% had increased chest tube drainage and blood product transfusion during the first 48 hours (153). The authors concluded that FXIII administration would be justified if levels after extracorporeal circulation are <70% (153).…”

“…Therefore, large volume transfusions of FFP would be necessary to effectively increase the fibrinogen concentration, and achieving the target fibrinogen concentration of 1.5-2.0 g/l is virtually impossible given the average fibrinogen concentration of 2 g/l. When replacing most FFP transfusions with fibrinogen concentrate, monitoring factor XIII levels is advisable after the replacement of approximately 50% of the blood volume; in addition, a 60% factor XIII activity level is to be maintained with the administration of factor XIII concentrate [35,[50][51][52].…”

“…In a randomised controlled trial of recombinant factor XIII vs placebo, there was no therapeutic benefit on transfusion avoidance, transfusion requirements or operative revision, despite an observed correction of factor XIII levels. Factor XIII administration outside clinical trials should therefore be discouraged [67][68][69][70][71].…”
