Colon Capsule Endoscopy vs. CT Colonography Following Incomplete Colonoscopy: A Systematic Review with Meta-Analysis

Abstract: Following incomplete colonoscopy (IC) patients often undergo computed tomography colonography (CTC), but colon capsule endoscopy (CCE) may be an alternative. We compared the completion rate, sensitivity and diagnostic yield for polyp detection from CCE and CTC following IC. A systematic literature search resulted in twenty-six studies. Extracted data included inter alia, complete/incomplete investigations and polyp findings. Pooled estimates of completion rates of CCE and CTC and complete colonic view rates (C… Show more

“…However, adjustments to the procedures will be pursued. The completion rate was lower than in previous studies 11 12 13 14 , although in line with results from other screening populations 4 15 and with a conclusive investigation rate of 80.3 %. The PDR was 73.5 %, whereas in the national CRC screening program, the adenoma detection rate (ADR) is 49 %.…”

“…A recent meta-analysis seeing completion rates between 65% and 93% (pooled 76%). 11 Colour cue software, such as the suspected blood indicator (SBI) mode has been shown to be extremely sensitive for active bleeding but less reliable for discrete lesions or ulceration, with physician review required for isolation of the cause of the bleeding once seen on SBI. 23,24 SBI software works by recognising clusters of red-coloured pixels, 25 and so the sensitivity can be changed, at the cost of specificity, by increasing the range of colour shades and the number of pixels accepted.…”

“…One such strategy to visualize the colon is by means of colon capsule endoscopy (CCE), which allows for a pain-free colonic assessment by eliminating the need for instrument insertion, gas insufflation or sedation [ 1 , 2 ]. This has allowed for the extension of CCE’s role in colorectal cancer (CRC) screening in the average-risk population, as well as an alternative for patients who refuse optical colonoscopy (OC) or in whom the latter is contraindicated [ 3 , 4 , 5 ]. Although CT colonography (CTC), like CCE, can visualise the colon without biopsy or therapy capabilities, in CTC, gas insufflation is required while concerns remain as to the ”allowed” frequency of use due to radiation exposure.…”
