Zahlreiche Neuigkeiten wurden im Jahr 2005 auf den drei für die deutsche Gastroenterologie wesentlichen Jahreskongressen prä-sentiert -neben der nationalen Tagung der DGVS in Köln auch der amerikanische (DDW Chicago) und europäische (UEGW Kopenhagen) Gastroenterologenkongress -die im Folgenden in einer natürlich notwendigerweise subjektiven Auswahl des Wesentlichen dargestellt werden sollen. Mukosektomie und erweiterte ResektionstechnikenJapanische Gruppen kontrollieren nach wie vor das Gebiet der endoskopischen Mukosaresektion (EMR) wie auch neue Techniken der endoskopischen Submukosadissektion (ESD), bei der mittels verschiedener Messer die Läsion umschnitten und dann schrittweise von der Unterlage abpräpariert wird. Im Gegensatz zu früheren Studien [1 -4] scheint mit zunehmender Erfahrung zumindest in Japan durch die ESD die Rate der En-bloc-und R0-Resektionen über 90 % zu liegen [5], sodass einer der Nachteile der EMR [6] dadurch behoben werden könnte. Eine ganze, nur aus japanischen Präsentationen bestehende Sitzung auf der UEGW war dem Thema ESD in Magen, aber auch Ösophagus und Kolon gewidmet [7 -20], Hier lagen die Erfolgsraten -Entfernung en bloc, d. h. in einem Stück und mit tumorfreien Rän-dern (R0) -bei den meisten, aber nicht bei allen Studien über 90 % bei einem mittleren Zeitaufwand von 1 Stunde [21]. Insgesamt scheinen diese Techniken, wiewohl generell schwer, im Magen etwas leichter als in Ösophagus und Kolon zu sein. Gruppen außerhalb Japans oder solche mit geringerer Fallzahl [1, 17, 22, 23] schlagen sich allerdings weniger wacker: Hier liegt die Erfolgsrate (vollständige Entfernung in einem Stück) schon mal unter 70 % und die Komplikationsrate bei 40 %, wenngleich den Abstracts zufolge nahezu alle Komplikationen, auch Perforationen, endoskopisch gelöst werden konnten. Auch deshalb darf allerdings bezweifelt werden, ob westliche Endoskopiker die Geduld einer angesichts der geringen Fallzahl im Magen langen Lernkurve aufbringen werden. Die totale Barrett-Mukosektomie wurde aus Hamburg [24] und Amsterdam [25]berichtet. Liegen eine (oder mehrere) hochgradige Dysplasien oder Frühkarzinome vor, so kann zusätzlich zur Abtragung der fokalen Läsionen der gesamte restliche Barrett abgetragen werden, um der Chirurgie vergleichbare Ergebnisse zu erzielen und die restliche Risikoschleimhaut zu entfernen. Nötig ist ein solches Vorgehen -will man bei der endoskopischen Resektion bleiben -, wenn in einem Barrett mehrere unsichtbare neoplastische Läsionen vorliegen. Beide oben erwähnte Gruppen verwendeten bei 29 und 30 Barrett-Patienten mit hochgradigen Dysplasien und Frühkarzinomen ein neues Ligatursystem, bei dem ohne Unterspritzung die Schleimhaut nacheinander (6-Ligatur-Set) ligiert und anschließend durch einen Kanal im Bandingset mit der Schlinge abgetragen wird. Another Spanish paper from Barcelona also matched colorectal cancer cases with previous colonoscopies and found 866 patients over 5 years, with 16 missed lesions (2.7 %) [63]. A nice idea from a British group was to place a clip once the cecum was reached (in the endoscopist's opinion) and then carry out a radiographic check on its position; this might be quite valid for quality control, but is not feasible (due to radia− tion).…”

Assessing the quality of the endoscopic abstracts accepted at the United European Gastroenterology Week (UEGW) meeting is al− ways necessarily subjective and may reflect the progress of en− doscopic research as well as peculiarities of the selection com− mittees. Nevertheless, the author's subjective impression for 2005 was that less original work primarily sent to UEGW was available. In fact, many large studies recently presented were not seen at UEGW ± e. g. papers on double−balloon endoscopy and reports on some new colonoscopes. Instead, a few new tech− niques were sometimes reported without clear−cut results, al− though the authors told us how happy they were. Let's hope this will change in the years to come. Reflux and Barrett's Complex; Early Cancers and ImagingSurveillance of Barrett's esophagus by conventional means, with close observation using modern video endoscopes and four− quadrant biopsies [1, 2], is obviously still thought to be so un− bearable and imperfect that many efforts are being undertaken to improve the situation and avoid the biopsies ± although the percentage of the daily workload taken up by Barrett's esopha− gus surveillance in an average endoscopy center is probably neg− ligible. Staining with various solutions [2, 3] has been suggested as one way of improving lesion detection (although the results have been quite variable [4], and narrow−band imaging (NBI) was developed to more or less replace staining [5, 6]. The Amster− dam group presented the results of a randomized cross−over study in 28 patients with Barrett's esophagus who were exam− ined with high−resolution endoscopy with either staining or NBI within 6 weeks, with no appreciable differences between the two groups [7]. NBI was also tested in the colon on various polyps, and different vascular patterns were described, although clear accuracy rates or correlation values with histology were not given [8].There are currently two ways of imaging early neoplastic lesions: lesion identification and tissue characterization (enthusiastically termed "endoscopic histology"). For identifying lesions, auto− fluorescence [9] is now available in a video format, and ± so far as I understand the abstract ± a group in Japan showed that auto− fluorescence identified five secondary lesions in patients sent for mucosectomy of early gastric cancer, whereas conventional en− doscopy found one [10]. Barrett's esophagus is another area for autofluorescence imaging [11]. In the colon, autofluorescence was used with and without NBI in a variety of lesions, but con− clusions regarding the rate of adenoma detection with autofluor− escence could not be made due to small numbers. NBI prediction of adenomas according to the Kudo classification achieved a sen− sitivity and specificity of 85 % and 90 %, respectively [12].The other area for imaging, tissue characterization, is represen− ted by various techniques such as confocal laser microscopic en− doscopy (CLME) [13 ± 15] and endocytoscopy [16,17].
