Metamorphopsia: An Overlooked Visual Symptom

Metamorphopsia is a common symptom in different macular disorders. Micropsia and macropsia are special types of metamorphopsia. Recent theories suggest that both retinal and cortical mechanisms are involved in the development and changes of metamorphopsia. Different functional tests have been proposed for the evaluation of metamorphopsia: from the Amsler grid to the hand-held mobile devices for home monitoring. This review addresses some new insights into the pathophysiology of metamorphopsia and different available tests for the evaluation of this symptom in most common macular disorders. The importance of quantification of metamorphopsia in macular diseases is confirmed by the most recent therapeutic approaches.

With modern surgical techniques to repair retinal detachments, a greater than 90% primary anatomic success rate can be expected. [1][2][3][4][5][6][7][8][9] Despite this high level of anatomic success, visual results remain compromised mainly because of permanent functional damage once the macula becomes detached. 10 The most important predictor of visual recovery after retinal detachment surgery is preoperative visual acuity. 8,9,11,12 Preoperative acuity appears to be directly related to the height of macular detachment. 13 Shorter duration of detachment and younger age are also important in visual recovery. This paper will review the current literature that helps to improve our understanding of visual recovery after macula-off retinal detachments.

Purpose To compare the macular functions of patients with excellent visual acuity after retinal detachment (RD) surgery, with the healthy fellow eye. Methods Of 214 patients, nine patients, who were successfully operated because of unilateral RD involving the macula and achieved excellent visual acuity, were analysed. The fellow eyes of the patients were taken as the control group. The macular functions were evaluated with visual acuity, contrast sensitivity, colour vision, visual field, and pattern VEP in operated and nonoperated eyes. Results There was no significant difference in visual acuity, contrast sensitivity, colour vision, and VEP outcomes between operated and nonoperated eyes. However, it was seen that the mean deviation in the visual field was significantly higher in the operated eyes (5.87 1.8 dB) when compared with the undetached fellow eyes (3.171.8 dB), (t ¼ 12.5; P ¼ 0.013). Conclusion Even though the visual acuity, colour vision, contrast sensitivity, and VEP results returned to normal after a successful RD surgery, we found that the mean deviation measured by the visual field, which reflects the retinal sensitivity, was still significantly low after nearly 5 years follow-up.
