Colpectomy Significantly Reduces the Risk of Urethral Fistula Formation after Urethral Lengthening in Transgender Men Undergoing Genital Gender Affirming Surgery

Abstract: Primary colpectomy decreases the incidence rate of urethral fistulas. Secondary colpectomy is also an effective treatment of fistulas at the proximal urethral anastomosis and the fixed part of the neourethra.

“…31 Even more, colpectomy is gaining in importance in preparation for genitoplasty with urethral elongation, as colpectomy has been shown to reduce the long-term urethral complications associated with genitoplasty. 31,32 There is no standardized approach to vaginectomy for transmasculine people. The most commonly described approaches have been: (1) transperineal total vaginectomy with colpocleisis; (2) robotic total vaginectomy with colpocleisis; or (3) transvaginal fulguration of the mucosa.…”

“…Al-Tamini et al compared 62 patients having undergone primary colpectomy to 232 patients who did not undergo colpectomy and found the rate of fistulae to be significantly lower in the colpectomy group (21% versus 48%; p < 0.01). 32 Furthermore, it is important to note that anterior vaginal-wall flaps harvested during vaginectomy may be utilized for construction of the proximal portion of the urethra during phalloplasty or metoidioplasty. 39…”

“…Urination from the tip of a neophallus, therefore, requires construction of both the pars fixa and pars pendulans urethra. Because of both the complexity of constructing the pars fixa urethra and the reported 30%-70% rate of subsequent urethral strictures and fistulae [27,58] , specific expertise is required to perform these procedures and manage the complications. Therefore, in centers performing phalloplasty with full urethral reconstruction, a collaborative team that includes expertise in reconstructive microsurgery and in reconstructive urology is essential.…”

“…Bij persisteren van de fistel wordt deze minimaal drie maanden na de operatie gesloten. Het optreden van urethrafistels in onze populatie is significant verminderd sinds er standaard voorafgaand aan de genitale GAO met urethraverlenging een colpectomie wordt verricht [7].…”
