Combination of Bernouilli effect producing maneuver-induced pharyngeal narrowing rate with body mass index as predictive tool for obstructive sleep apnea syndrome

Abstract: The criterion of BEPM at a cut-off value of 50% with BMI at a cut-off value of 25 kg/m(2) achieved a sensitivity of 93%, while the specificity was 67% in differentiating simple snorers from OSAS patients (apnea index >5). In addition, the likelihood ratio of the method was found to be 2.81.

“…However, when using a 50 percent collapsibility as a cutoff point, the authors showed that the method provided only a sensitivity of 67 percent and a specificity of 83 percent for the identification of OSAS patients among snorers. Later, 15 the same authors adjusted their results by including BMI, which improved their sensitivity and specificity to 93 percent and 67 percent, respectively. We believe that the aforementioned methods, although novel, are somewhat impractical and still involve a considerable degree of subjectivity.…”

“…In OSAS, besides respiratory disorders in sleep, serious complications occur in other systems, especially in the cardiovascular system, and these complications constitute the most important cause of mortality and morbidity in OSAS. With the prolongation of lifetime, the frequency of chronic diseases increases: 90% of people aged 65 and over have at least one chronic disease, while 35% of them have 2, 23% of them have 3, 15% of them have 4 or more diseases simultaneously [7].…”

“…OSAS is a disease coursing with recurrent apnea and hypopnea episodes, that develop due to obstructions in one or more areas of the upper respiratory tract during sleep [6]. It has been reported that disruption between the neural and muscular factors responsible for keeping the airway open during sleep causes obstructions [7]. This can range from partial obstruction of the airway that causes simple snoring, to full obstruction of the airway that causes apnea formation.…”

“…Uyku süresince hava yolunu açık tutan nöral ve musküler faktörler arasındaki dengenin bozulması-nın, obstrüksiyonlara neden olduğu bildirilmiştir (1). OSAS tedavi edilmediği takdirde kardiyovasküler ve serebrovasküler morbidite ve mortalitede artışa neden olan, nöro-kognitif disfonksiyona yol açan ve yaşam kalitesini düşüren kronik bir hastalıktır (2).…”
