Combined 30-degree bevel up and down technique against 0-degree phaco tip for phacoemulsification surgery of hard cataracts

Abstract: PurposeTo evaluate the effective phaco time (EPT), cumulative dissipated energy (CDE) and nucleus emulsification time (NET) as phaco parameters with 0- and 30-degree phaco tip.Patients and methodsThis prospective, interventional and observational case series included 294 patients scheduled for cataract removal by phacoemulsification technique. Patients with nuclear cataracts of grade 4 and 5 nuclear opalescence of Lens Opacities Classification System III were included in the study. Patients were consecutively … Show more

“…In the literature, the ultrasonic energy expended during phacoemulsification is expressed as two different but related values, namely, EPT and cumulative dissipated energy (CDE). These metrics are sometimes confused [14,24,76,77]. EPT is automatically calculated by Stellaris (our phacoemulsification machine; Bausch & Lomb) and Sovereign WhiteStar (Advanced Medical Optics).…”

“…Thus, competent ophthalmologists capable of performing cataract surgery without any complications are highly required. Most of the cataracts in the Asian population are hard cataracts because patients generally present late for surgery [4][5][6] . Manual small-incision cataract surgery (MSICS) is the preferred technique for cataract removal in developing countries [7][8] .…”

“…However, they did not compare the effect of these manoeuvres on corneal endothelial cell loss. Hence, there is no consensus regarding the most cornea friendly phaco tip position during phacoemulsification 28 .…”
