Combined Embolization and Operation in the Treatment of Cervical Arteriovenous Malformations

Abstract: The effectiveness of a combined approach of embolization and operation for cervical vascular malformations is stressed. Lesions that seem to be inoperable may have their blood flow sufficiently reduced by preoperative embolization to allow surgical extirpation. Polyvinyl alcohol foam is an ideal embolic agent; not only may it be used preoperatively, but its permanency allows it to be used for primary therapy.

“…Experience with craniofacial AVM shows a high success rate is obtaining satisfactory devascularization prior to surgery thereby reducing operation time and blood transfusion [20,21]. AVMs in certain inoperable sites offer good palliation and arrest the progress of the disease following embolization (Fig 3).…”

“…Surgery has been the traditional mode of therapy for cervical arteriovenous fistulas [3,4,5,10,12]. The problems of surgical treatment, i.e., intraoperative hemorrhage, incomplete closure of the fistula, and the generally difficult access to the upper cervical region, are well known.…”

“…14) Treatment has evolved considerably since 1990, and endovascular occlusion of fistulas is now the main option of the treatment. [6][7][8] In the beginning of the endovascular era, detachable balloons were used as the material of the embolization.…”
