Combined laparoscopic and robotic surgery for synchronous colorectal and genitourinary cancer: A case series

Abstract: HighlightsCombined laparoscopic and robotic surgery for synchronous colorectal and genitourinary cancer are extremely rare.In simultaneous endoscopic surgery, it is necessary to decide sequence of resection, intraoperative patient’s position and port arrangement.Combined laparoscopic and robotic surgery is suitable for advanced cancer cases requiring multidisciplinary treatment.

“…Imagami et al retrospectively analyzed the surgical outcomes from a series of 3 cases of laparoscopic colorectal resection combined with robotic partial nephrectomy (n 1) or radical prostatectomy (n 2). They stated that the minimally invasive combined surgery was safe and feasible and allowed prompt initiation of adjuvant chemotherapy [17]. In our case, we used five ports confirming that for synchronous colorectal and urogenital tumor the sharing ports are possible also by robotic approach.…”

“…The advantage to treat two neoplasms at the same time is to avoid a delay treatment of second tumor, especially in younger patients [16] . Finally, a combined surgery reduces the time to start the post-operative adjuvant chemotherapy that is longer if two different procedures are performed separately [17]. Moreover, a second anesthetic procedure may be avoided, and discomfort of patients could be reduced [18].…”

“…With the spread of robotic surgery, there have also been reports on simultaneous surgery using robotic surgery and laparoscopic surgery, and their usefulness has been reported. On the other hand, the importance of the port arrangement at that time has also been reported [ 5–7 ].…”

“…However, on the other hand, there have also been reports of RARP being performed first in cases where the robotic and laparoscopic procedures were performed concurrently. This is because the ureter was easily seen on the inner approach and dissection of the ventral aspect of the rectum was not necessary [ 5 ].…”

“…In terms of technical feasibility and oncological safety, simultaneous laparoscopy combined with resection is the best option for patients with co-existing abdominal lesions [7]. We have previously reported about the advantages of combined laparoscopic and robotic resection for synchronous colorectal cancer and PCa [8]. Based on such experience, combined LTG and RARP were selected.…”
