Combined Serum Proteomic and Metabonomic Profiling After Laparoscopic Sleeve Gastrectomy in Children and Adolescents

Abstract: LSG leads to changes in amino acids and in lipid metabolism indicated by glycerol-phosphatidylcholines and SM. The pattern of protein biomarkers and metabolites might provide measures for the induced physiological changes and for therapy monitoring.

“…The regulation of RBP-4 is unclear, however RBPR2 was recently identified as a high affinity RBP4 receptor expressed primarily in liver and small intestine and induced in adipocytes of obese mice [21]. Studies show that RBP4 is decreased with hypocaloric diet [22] as well as weight loss achieved by lifestyle [23] or weight loss achieved by SG bariatric surgery [12], [24].…”

“…New opportunities for discovery in biofluids have also opened up with the development of selective depletion strategies for highly abundant proteins which otherwise tend to dominate the analysis, obstructing the analysis of biomarkers and other key proteins that are often in low abundance [11]. Complementing proteomic techniques, metabolomic analysis offers a highly sensitive means to profile a large number of metabolites in various metabolic pathways [12] and provides useful information on activity of metabolic pathways e.g. lipid and carbohydrate metabolism [13], [14].…”

“…However, it has been shown that bariatric surgery caused differential metabolic changes in AA profiles of patients with obesity compared with dietary intervention, despite identical weight loss of the subjects. 20,21 Additionally, only a few studies investigated the effect of weight reduction on the AA levels in time points shorter than 1 year. An intriguing issue is the dynamics of changes in the metabolome of people with obesity during weight loss.…”

“…25,26 The increased plasma BCAA concentration may be the result of the impaired BCAA-catabolizing capacity of the adipose tissue, an improper high-fat and highprotein diet (dietary protein comprised of >20% BCAA) and increased catabolic pathways of BCAAs. 13,20,27,28 So far, only a few studies rated the changes in the BCAA levels after weight loss through diet and physical activity, but those studies were conducted on different populations (especially Caucasian and Japanese subjects) and used another model of body mass reduction program (based on nutritional recommendation and lifestyle intervention, but not a strictly composed diet). A 7-year prospective KORA study on a group of Caucasian patients with obesity showed a significant correlation between the average percentage increase in body weight and the increase in the BCAA, phenylalanine, tyrosine, and glutamate levels.…”

“…Finally, as mentioned above, data in adolescents (Nadler et al 2012) and long-term data in adults remain scarce (Oberbach et al 2012;Till et al 2008;Alqahtani et al 2012a, b;Boza et al 2012;Bohdjalian et al 2010;Rosenthal et al 2012). While gastric banding and gastric bypass can be reversed to preoperative anatomy, this cannot be done after sleeve gastrectomy due to definitive resection of the excess stomach.…”
