Despite significant therapeutic advances, the toxicity of conventional therapies remains a major obstacle to their application. Radiation therapy (RT) is an important component of cancer treatment. Therapeutic hyperthermia (HT) can be defined as the local heating of a tumor to 40–44 °C. Both RT and HT have the advantage of being able to induce and regulate oxidative stress. Here, we discuss the effects and mechanisms of RT and HT based on experimental research investigations and summarize the results by separating them into three phases. Phase (1): RT + HT is effective and does not provide clear mechanisms; phase (2): RT + HT induces apoptosis via oxygenation, DNA damage, and cell cycle arrest; phase (3): RT + HT improves immunological responses and activates immune cells. Overall, RT + HT is an effective cancer modality complementary to conventional therapy and stimulates the immune response, which has the potential to improve cancer treatments, including immunotherapy, in the future.

Temperature distribution is an important factor in thermo-radiotherapy and it is greatly dependent on the applied heating technique. Consistency of the heating method is therefore important in translating in vivo experimental data to the clinical situation. To further evaluate the combination of interstitial hyperthermia and interstitial radiotherapy, an experimental interstitial hyperthermia system has been developed for small (500-2000 mm3) tumours growing in the flank of a rat. The system used reproduces the properties of our clinical current source interstitial hyperthermia system. The heating system consists of four applicators, each with independent tuning and power control. The applicators are situated inside plastic afterloading catheters and are capacitively coupled with the surrounding tissue. The tumour is heated through dissipation of a 27 MHz current flowing to an external ground plane. An effective RF-filter allows reliable thermocouple temperature measurements when the power is switched on. The tumour temperature is easily controlled by means of a continuous temperature read-out and a clear temperature display. A minimum temperature up to 46 degrees C can be reached within 4-10 min and maintained (+/-0.5 degrees C) throughout the treatment period. Modelling calculations performed for this heating system indicate that the applicator temperatures should be kept equal in order to minimize the difference between maximum and minimum temperature. Significantly higher applicator currents are needed at larger distances from the ground plane. In addition, the homogeneity of the temperature distribution is improved when either the tumour is isolated or when the environmental temperature is increased. The calculations also show that temperature distribution is strongly dependent on effective heat conductivity. A description of the system and its performance is presented.

The combination of interstitial hyperthermia treatment (IHT) with high dose rate brachytherapy (HDR-BT) can improve clinical outcomes since it highly enhances the efficiency of cell kill, especially when applied simultaneously. Therefore, we have developed the ThermoBrachy applicators. To effectively apply optimal targeted IHT, treatment planning is considered essential. However, treatment planning in IHT is rarely applied as it is regarded as difficult to accurately calculate the deposited energy in the tissue in a short enough time for clinical practice. In this study, we investigated various time-efficient methods for fast computation of the electromagnetic (EM) energy deposition resulting from the ThermoBrachy applicators. Initially, we investigated the use of an electro-quasistatic solver. Next, we extended our investigation to the application of geometric simplifications. Furthermore, we investigated the validity of the superpositioning principle, which can enable adaptive treatment plan optimization without the need for continuous recomputation of the EM field. Finally, we evaluated the accuracy of the methods by comparing them to the golden standard Finite-Difference Time-Domain calculation method using gamma-index analysis. The simplifications considerably reduced the computation time needed, improving from >12 h to a few seconds. All investigated methods showed excellent agreement with the golden standard by showing a >99% passing rate with 1%/0.5 mm Dose Difference and Distance-to-Agreement criteria. These results allow the proposed electromagnetic simulation method to be used for fast and accurate adaptive treatment planning.
