Combining default choices and an encounter decision aid to improve tobacco cessation in primary care patients: protocol for a cluster-randomized trial

Abstract: Background While quitting smoking dramatically decreases overall mortality, general practitioners (GPs) are less likely to prescribe medications for smoking cessation than other cardiovascular risk factors. Guidelines recommend providers first assess patients’ “readiness” to quit, an “opt-in” strategy, but only a minority of tobacco users are ready to quit on a given day. An “opt-out” strategy offering treatment as the default choice increased quit attempts in hospital and with pregnant women,… Show more

“…Nudge theory can be used to change the default option, such as making non-smoking or non-drinking the default option in certain situations. Hempel-Bruder et al (39) evaluated the effectiveness of educating General Practitioners (GPs) to provide treatment as the default option with current tobacco users seen in primary care using an encounter decision aid. The use of default options and an electronic decision aid are low-cost, readily disseminable interventions.…”

“…The use of default options and an electronic decision aid are low-cost, readily disseminable interventions. They hypothesized that general practitioners who provide smoking cessation treatment as the default option using an encounter decision aid will boost the percentage of patients who cease smoking (39).…”

“…Effective methods of smoking cessation include pharmacological therapy [11], nicotine replacement therapy [12], and various psychological therapies such as counseling [13,14] and cognitive behavioral therapy [15,16]. Combining these pharmacological or nicotine replacement therapies with psychological therapy has been found to be the most effective [17,18], and many habitual smokers are attempting to quit on their own, one way or another.…”
