Combining the serum pepsinogen level and Helicobacter pylori antibody test for predicting the histology of gastric neoplasm

Abstract: A combination of serum PG levels and H. pylori antibody test is useful for detecting gastric neoplasms based on the slow gastric carcinogenesis pathway progressing from gastric adenoma to Lauren's intestinal-type gastric cancer, but not for those with advanced histology such as Lauren's diffuse-type gastric cancer.

“…pylori (2) group (4.17 %; odds ratio 25.8, P50.009), but the neoplasm exhibited the least advanced histology using the IgG serology test. Importantly, in this previous study we could not confirm whether there was a difference between Western and Korean serology kits (Choi et al, 2014) because the Genedia H. pylori ELISA test, which utilizes H. pylori antigen obtained from Korean strains, was not tested. In our current study, we found a high concurrence rate among the three serological tests with each displaying similar diagnostic accuracies.…”

“…We recently showed that serum anti-H. pylori IgG antibodies are also useful in assessing the risk of gastric neoplasia and predicting pathology type in Korean adults (Choi et al, 2014). Of the 3328 participants included in this previous study, the incidence of gastric neoplasia was highest in the gastric atrophy (+)/H.…”

“…The samples were centrifuged and stored at −70 o C before analysis. Serum PG I and PG II concentrations were analyzed using the latex-enhanced turbidimetic immunoassay (HBi, Anyang, Korea) as described (8). …”

“…The serum PG assay estimates the secreting ability of the gastric mucosa cells, and therefore, it is useful when selecting asymptomatic subjects for endoscopy surveillance before and after the resection of gastric neoplasm (78910). Nonetheless, there are still controversies with regard to the link between the serum PG assay findings and abnormal upper gastrointestinal (UGI) endoscopic findings.…”

“…A longitudinal study conducted in China, a high-risk of gastric cancer zone, has established that increased levels of PGI, PGII, anti-H. pylori IgG and a decreased PGI/II ratio is associated with the progression of gastric precancerous lesions [14].…”
