Aim:To determine and compare the treatment outcome between symptomatic and asymptomatic impacted mandibular third molars in the same patients who presented with bilateral impacted mandibular third molars. Methods: This preliminary study was done at the Dental and Maxillofacial Surgery Clinic of our institution over a period of 18 months. The variables analysed were age, gender, socio-economic class, duration of symptoms before treatment, types of impaction and complaints during the follow-up. Descriptive and bivariate statistics were computed and the P value was set at 0.05. Results: The sample was composed of 27 patients, and their ages ranged from 18 to 41 years with a mean age of 31 AE 2.3 years. There were more males than females, with male: female ratio of 1.1:1. The complication rate was 8 (29.6%) in 5/27 (18.5%) patients under symptomatic wisdom teeth impaction while in asymptomatic impacted teeth, the rate was 2 (7.4%) in 2/27 (7.4%) patients and this difference was significant (P = 0.02). The complications occurred only in patients in the lower socio-economic class (P = 0.01) and in symptomatic cases, the longer the duration of symptoms before treatment, the more the complications (P = 0.001). The complications following treatment were alveolar osteitis, numbness of the ipsilateral lower lip and hypersensitivity of the adjacent second molar. Conclusion: This study shows that the surgical extractions of symptomatic and asymptomatic impacted mandibular third molars were associated with post-operative complications that were more in the symptomatic cases. The findings in this study may have to be investigated further in a future study. Postoperative pain was suffered by 80% of the total patients. One of the many surgical procedures performed is orthopedics. Ketorolac and fentanyl are often used for postoperative orthopedic pain. Appropriate postoperative pain management with adequate analgesic administration will accelerate pain reduction and shorten the patient's hospital stay. This study aims to determine the difference in the effect of continuous fentanyl and intravenous ketorolac on pain targets achievement and Length of Stay (LOS) of patients after orthopedic surgery. This study is a prospective cohort study, involving inpatients after orthopedic surgery in Dr. Sardjito Yogyakarta in July-October 2019. The independent variable is continuous fentanyl and intravenous ketorolac, while the dependent variable is the achievement of pain targets and LOS. The analysis was conducted by Fisher's exact test, Mann Whitney's test, and multivariate analysis to determine the dominance of variables on the research outcomes. There were 59 patients after orthopedic surgery in this study. A total of 34 patients in the continuous fentanyil group and 25 patients in the intravenous ketorolac group. There was no difference in the effect of continuous fentanyl administration and intravenous ketorolac on pain targets achievement (P = 0.711), while there were differences in the effect of continuous fentanyl administration and intravenous ketorolac on the average LOS of patients in the hospital (P = 0,000). Continuous fentanyl and intravenous ketorolac can both produce the achievement of adequate postoperative orthopedic pain targets, but the mean LOS becomes shorter in patients with intravenous ketorolac.

Loxoprofen Sodium Versus Diclofenac Potassium for Post-Dental Extraction Pain Relief: A Randomized, Triple-Blind, Clinical Trial

One of the most common post-operative complications of tooth extraction is pain. Oral analgesics, namely loxoprofen sodium and diclofenac potassium, are often prescribed; however, the efficacy of these drugs irrespective of gender and type of extraction has not been tested. Therefore, this study aimed to compare the efficacy of these two drugs in post-dental extraction pain relief among male and female patients in cases of simple and surgical tooth extraction. A single-center, triple-blind, randomized clinical trial was conducted among 100 male and female patients who underwent tooth extraction at Taibah University Dental College and Hospital in Al-Madinah, Saudi Arabia. The patients reported their pain post-operatively after 6 hours and every 12 h for 3 days using the Verbal Descriptor Scale (e.g., “no pain”, “mild pain”). Descriptive statistics and chi-square tests were run to analyze the data. An equal number of patients received either the drug loxoprofen sodium or diclofenac potassium and completed the study follow-up. Patients allocated to the diclofenac potassium drug group after 36 h were statistically significantly in their reporting of “no pain” and “mild pain” compared to patients allocated to the loxoprofen sodium group (86% vs. 66%, respectively; p = 0.019), irrespective of gender or type of tooth extraction. However, both groups demonstrated comparable (p > 0.05) post-operative pain relief over the other aforementioned allocated time intervals. In conclusion, the diclofenac potassium group had slightly better control over post-operative pain than the group receiving loxoprofen sodium.
