Ações educativas, cuidados ambientais e tratamento farmacológico são condutas necessárias ao tratamento da asma. O objetivo deste trabalho foi aplicar técnica que ajude a compreensão de asmáticos quanto ao uso dos medicamentos. Trata-se de um relato de experiência, realizado em uma Unidade Básica de Saúde em Santa Maria, Distrito Federal, de março a agosto de 2022, no qual o Arco de Maguerez foi o instrumento metodológico. Foram desenvolvidas etapas de estudo, sendo a primeira, a observação da realidade, identificando a dificuldade de entendimento de pacientes sobre o uso dos antiasmáticos. Na segunda etapa, foram elencados os pontos-chave para o problema, sendo o principal, a falta de informação desde a consulta com o profissional de saúde até a dispensação do medicamento. As etapas anteriores foram teorizadas durante a terceira etapa do estudo. A produção de um folder educativo, a organização de um fluxo de atendimento, a educação continuada dos técnicos da farmácia e o matriciamento dos prescritores foram hipóteses de soluções avaliadas na quarta etapa. Por fim, na última etapa, foi preparado um folder com as principais informações para uso correto de antiasmáticos e seus dispositivos. Dessa forma, a solução escolhida é uma intervenção farmacêutica capaz de auxiliar o paciente em uma farmacoterapia racional.

The Association Between Short-Acting β2-Agonist Over-Prescription, and Patient-Reported Acquisition and Use on Asthma Control and Exacerbations: Data from Australia

Introduction In Australia, short-acting β 2 -agonists (SABA) are available both over the counter (OTC) and on prescription. This ease of access may impact SABA use in the Australian population. Our aim was to assess patterns and outcome associations of prescribed, acquired OTC and reported use of SABA by Australians with asthma. Methods This was a cross-sectional study, using data derived from primary care electronic medical records (EMRs) and patient completed questionnaires within Optimum Patient Care Research Database Australia (OPCRDA). A total of 720 individuals aged ≥ 12 years with an asthma diagnosis in their EMRs and receiving asthma therapy were included. The annual number of SABA inhalers authorised on prescription, acquired OTC and reported, and the association with self-reported exacerbations and asthma control were investigated. Results 92.9% ( n = 380/409) of individuals issued with SABA prescription were authorised ≥ 3 inhalers annually, although this differed from self-reported usage. Of individuals reporting SABA use ( n = 546) in the last 12 months, 37.0% reported using ≥ 3 inhalers. These patients who reported SABA overuse experienced 2.52 (95% confidence interval [CI] 1.73–3.70) times more severe exacerbations and were 4.51 times (95% CI 3.13–6.55) more likely to have poor asthma control than those who reported using 1–2 SABA inhalers. Patients who did not receive SABA on prescription (43.2%; n = 311/720) also experienced 2.71 (95% CI 1.07–7.26) times more severe exacerbations than those prescribed 1–2 inhalers. Of these patients, 38.9% reported using OTC SABA and other prescription medications, 26.4% reported using SABA OTC as their only asthma medication, 13.2% were prescribed other therapies but not SABA OTC and 14.5% were not using any medication. Conclusion Both self-reported SABA overuse and zero SABA prescriptions were associated with poor asthma outcomes. The disconnect between prescribing authorisation, OTC availability and actual use, make it difficult for clinicians to quantify SABA use. Performances and Activities of Virtual Pediatrics Medication Counselling Clinic in Riyadh City, Saudi Arabia

Objectives: To explore the activities and performances of a virtual pediatrics medication counseling clinic in Riyadh City, Saudi Arabia. Methods: it was a retrospective analysis of activities and performances of a virtual pediatric medicine counseling clinic operated by the pharmacist at a public pediatric and maternity hospital. The pharmacist documented all the activities, including demographic information of patients visiting the clinic, the reason for the counseling, medication requirements to be counseled, and patient counseling items based on international guidelines. The data analysis was done through SPSS version 20 and Microsoft Excel version 2021; descriptive analysis and logistic regression of factors associated with the reason for counseling. Results: The total number of patients who visited the virtual pediatric medicine clinic through 20 months was 1955, with (97.75) per month. The majority of patients were female in the year 2021, 204 (63.55%) vs. female 900 (61.19%) in 2021, compared to male 117 (36.46%) vs. males in year 2022 628 (38.81%) with non-significant differences (p=0.149). The majority of patients were diagnosed as Diabetic Miletus in both years, 145 (45.17%) vs 884 (54.47%), with statistical significance differences between two 2021 and 2022 (p=0.000). The majority of patients counseled due to high alert medication was 230 (71.65%) in 2021 vs. 1198 (73.32%) in 2022, with non-statistically significant difference between them (p=0.539), and chronic diseases such as diabetes and asthma 224 (69.78%) vs. 901 (55.14%) in the year 2021 and 2022 respectively with statistical significance differences between both years (p=0.000). Most medication counseled was Insulin Aspart in both years, 121 (21.72%) vs. 909 (31.92%), insulin Glargine 104 (18.67%) in 2021 vs. 555 (19.49%) in 2022, and Enoxaparin in both years 40 (7.18%) vs. 205 (7.20%). Conclusion: The virtual pediatric medicine counseling clinic served many patients, emphasizing pediatric chronic diseases such as Diabetes and Asthma. Future studies are highly recommended to measure the clinical and economic outcomes and patient satisfaction.
