“…Indeed, some ED-based care coordination interventions have been shown to benefit older patients, while others have not. 26 Transitions programs like those developed by Coleman et al, 5,6 which includes the creation of a personal health record and the initiation of a series of visits and telephone calls by a transitions coach, require a significant investment in infrastructure and may be beyond the ability of most EDs to implement. However, many EDs already have a staff member responsible for calling discharged patients with urgent test results.…”

“…Harding et al 210 found reductions in all three when they examined triage with management options against triage with prioritisation only. Katz et al 213 found that interventions that developed post-ED plans were generally effective in improving follow-up rates or reducing repeat visits to ED. Porter 216 found the same effect when examining rapid assessment models.…”

“…In the absence of a risk of bias assessment, it is more difficult to assess the strength of the evidence underlying the review's conclusions. Seventeen reviews 204,205,[207][208][209][210][211][212][213][215][216][217][218][219][220]222,224 were judged to have used an appropriate method to synthesise the studies. In most cases a narrative approach was used because the included studies were too heterogeneous for meta-analysis.…”

“…7 Transitions of care are a key aspect of care coordination, and the role of the ED in facilitating such efforts needs to be determined. As highlighted by Katz et al, 8 it has to be established whether the ED plays a central (ie, ED-centric care coordination) or a peripheral (ie, care coordination programs managed by longitudinal care workers outside the ED) role. Formulating the role of the ED within the care coordination model can only be ascertained through a comprehensive understanding of the impact of ED-based interventions.…”
