Amaç: Bu çalışmada romatoid faktörün (RF), anti-siklik sitrülin peptit (anti-CCP) antikorunun ve anti-mutat sitrülin vimentin (anti-MCV) antikorunun iki farklı ticari kitinin, ticari kitlerin performansları arasında önemli farklılıklar bulunabileceğini de göz önünde bulundurarak, romatoid artritteki (RA) tanısal değerleri test edildi. Bulgular:Otoantikorların tanısal duyarlılık ve özgüllüğünü belirlemek için yapılan alıcı işletim karakteristiği (ROC) eğrisi analizinde en yüksek eğri altında kalan alan değeri anti-CCP-2'ye aitti. Duyarlılık ve özgüllük RF için %88 ve %90, anti-CCP-2 için %84 ve %100, anti-MCV-548 için %80 ve %90 ve anti-MCV-248 için %84 ve %100 idi. DAS-28 ile anti-CCP-2 (r= 0.623), anti-MCV-548 (r= 0.481) ve anti-MCV-248 (r= 0.408) arasında zayıf bir korelasyon vardı. Ekstraartiküler tutulumu olan ve olmayan hastaların ve DAS-28'e göre düşük veya orta şiddette hastalık aktivitesi olan hastaların RF, anti-CCP-2, anti-MCV-548 ve anti-MCV-248 değerleri arasında istatistiksel olarak anlamlı bir fark yoktu.

Abstract:The aim of this work was to assess the diagnostic value of anti-CCP-3 and anti-CCP-2 for the diagnosis of rheumatoid arthritis (RA) and determine whether anti-CCP-3 more accurately identifies patients with rheumatoid arthritis than anti-CCP-2. PubMed and CNKI databases were searched for studies published in English and Chinese that examined the use of anti-CCP-3 and anti-CCP-2 in the diagnosis of known or suspected rheumatoid arthritis from January 2006 to July 2013. Seventeen included studies of methodological quality were rated by using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS) tools A randomeffects method was used to summarize sensitivities, specificities, positive and negative likelihood ratio (LR+ and LR-, respectively), and diagnostic odds ratio from 17 studies. The pooled sensitivity, specificity, LR+, LR-and diagnostic odds ratio for anti-CCP-3 were 0.737 (95% CI, 0.717-0.757), 0.933 (95% CI, 0.924-0.942), 11.096 (95% CI, 0.274 (95% CI, and 42.908 (95% CI,, respectively. For anti-CCP-2, the values were 0.719 (95% CI, 0.699-0.739), 0.960 (95% CI, 0.953-0.966), 17.485 (95% CI, 0.294 (95% CI,), respectively. With high specificity and moderate sensitivity, anti-CCP-2 and anti-CCP-3 played an important role in confirming the diagnosis of RA. Anti-CCP-3 did not have better diagnostic performances than anti-CCP-2, but anti-CCP-2 had evident heterogeneity compared to anti-CCP-3, especially in American patients.

at least 6 months of therapy with disease-modifying antirheumatic drugs (DMARDs; methotrexate, leflunomide, cyclosporine, chloroquine) and/or at least 6 months of therapy with anti-TNF biologics. The evaluation of response to RTX was made at month +6 using the EULAR criteria (DAS28). a-CCP, a-MCV, total RF and RF IgA were determined at baseline (before the first infusion of RTX) and after 1, 3 and 6 months. In serum samples obtained before treatment two cytokines essential for Blymphocyte proliferation, interleukin 6 (IL-6) and B-lymphocyte stimulator (BLyS) were also determined. In all patients a significant and consistent reduction in all the tested antibodies was found during follow-up, with no differences in respect of the degree of response to RTX. Of note, at baseline, generally a higher titre of all autoantibodies was seen in patients who then showed a better response to RTX. Finally, there were no differences in serum concentrations of IL-6 and BLyS in patients in relation to the presence or absence of the autoantibodies investigated, nor was there any significant correlation between the serum concentrations of the cytokines and the titres of the autoantibodies. Thus, neither a-MCV compared to a-CCP, nor RF IgA compared to routine total RF, provided any additional predictive information in the follow-up of patients with RA treated with RTX.
