Comparative Radiographic and Clinical Outcome of Two-Hole and Multi-Hole Symphyseal Plating

Abstract: In this group of patients, the two-hole symphyseal plating technique group had a higher implant failure rate and, more importantly, a significantly higher rate of pelvic malunion. On the basis of these findings, we recommend multi-hole plating of unstable pubic symphyseal disruptions.

“…Symphyseal disruption, without soiled open wounds or compromised soft tissues, is often best treated with open plate fixation. Despite the fact that blood loss may be significant in the coagulopathic trauma patient, and fixation failure may occur in up to 20% of patients, 19 this has remained the treatment of choice for this anterior pelvic fracture pattern. When the anterior injury pattern includes pubic rami fractures, particularly ''high'' fractures near the pubic root, surgeons may opt for no anterior pelvic fixation.…”

“…B. der Symphyse und/oder der Iliosakralfuge mittels Symphysenplatten und/oder transiliosakralen Schrauben, erforderlich war. Aufgrund der physiologischerweise auftretenden belastungsabhängigen Kräfte in der Symphyse kann durch die Osteosynthese nur für eine gewiss Zeit eine relative Stabilität erreicht werden [23]. Daher kommt es erwartungsgemäß bei über 30% der Patienten innerhalb eines Jahres zu einer Auslockerung bzw.…”

“…6 Recently, Sagi and Papp showed in a retrospective clinical study that fixation of the symphysis with a two-hole plate construct resulted in significantly more fixation failure and malunion when compared with a multihole plate construct. 9 Consequently, uniplanar multihole fixation has been the mainstay for plating of a pubic symphyseal disruprtion. 10 The pubic symphysis has inherent physiological motion.…”
