Comparative Sensitivity of Urinary Cyfra 21-1, Urinary Bladder Cancer Antigen, Tissue Polypeptide Antigen and Nmp22 to Detect Bladder Cancer

Abstract: Our preliminary evaluation showed the tests to be potential noninvasive adjuncts to help determine the need for cystoscopy. The combination of 2 tumor markers, NMP22 and 1 cytokeratin (CYFRA 21-1 or urinary bladder cancer antigen), seemed to be the most effective. Further comparative studies are needed to assess the promising diagnostic role of these markers.

“…18 The interference of leukocytes with urine-based tests has been noted by others, 37,47,48 and excluding affected subjects is a common way of circumventing this problem. 36,37 In our series, the specificity of hTR quantification is greatly improved by excluding leukocyte contamination in urine. In its presence, bladder cancer may still be detected with sufficient specificity by applying a higher cutoff value for hTR analysis.…”

“…Another version, called the urinary bladder cancer (UBC) test (IDL), also detects CK8 and 18. A preliminary report suggests a sensitivity of 65% and specificity of 92% for this test (276,278 ). In one method comparison study, the UBC test outperformed the BTA Stat and NMP22 tests, showing higher sensitivity and specificity for bladder cancer (279 ).…”

“…Although the overall sensitivity is reported to be 80%, a false-positive rate of 30%-40% has limited TPA use in routine patient care (276 ). Subsequently, a tissue polypeptide-specific (TPS) test (IDL Biotech) was developed, which employs monoclonal antibodies against CK8 and 18 (277 ).…”
