Comparative Study of Internal Fixation of the Ulna and Distal Ulna Resection in Patients Older Than 70 Years With Distal Radius and Distal Metaphyseal Ulna Fractures

Abstract: The results from our study show that women older than 70 years with fracture of the distal radius and distal ulna have a higher rate of complications if ORIF of the distal ulna is performed. Patients should be warned, by surgeons, of this in cases where ORIF of the distal ulna is suggested.

“…It is clear that it is not American authors who have resistance or difficulty in declaration of LOE. While the current review is focused on the J Hand Surg Am and J Hand Surg Eur publications, several articles have been published reporting the LOE in other journals (Andersson et al., 2018; Boretto et al., 2019; Boyer et al., 2019; Ehrl et al., 2016; El Amiri et al., 2019; Frueh et al., 2014; Fujihara et al., 2018; Goorens et al., 2018; Hohendorff et al., 2017; 2018; Huttin et al., 2018; Kamphuis et al., 2019; Karakaplan et al., 2019; Ma et al., 2017; Mulders et al., 2017; Rijnja et al., 2017; Robinson et al., 2017; Tang et al., 2017; Thorkildsen and Rokkum, 2019; Uluyardimci et al., 2019; van Zyl et al., 2019; Wang et al., 2017; Yeo et al., 2015). “…In selected cases a salvage procedure might be appropriate as primary management. A retrospective study compared the Darrach procedure with internal fixation in 24 women over the age of 70 years (Boretto, Zaidenberg et al 2019). They noted more complications in the internal fixation group including ulnar nerve palsy, non-union, plate loosening, infection, complex regional pain syndrome, pain, and restricted motion.…”

“…In the past 25 years, one of the most exciting areas in hand surgery has been the study of anatomy, biomechanics, and pathophysiology at the distal end of the ulna [ 4 ]. Ulnar head fracture may be seen in up to 6% of patients with unstable fractures of the distal radius [ 5 ], and this rate is ever increasing due to the growing proportion of elderly people [ 6 ]. Metaphyseal distal radial fractures associated with distal ulnar shaft fractures represent an unstable injury pattern, which may cause nonunion of the distal radius [ 7 ].…”
