Comparing Apples with Apples in Clinical Populations: Applications of the Adjusted Clinical Group System in British Columbia

Abstract: This article reviews the Adjusted Clinical Group Case-Mix System and describes how it is being applied in the management of physician services in British Columbia. Developed in the United States for management and research, adjusted clinical groups are used to measure the illness burden and health service needs of individuals and, when aggregated, of populations, by grouping the range of conditions coded on physician claims and hospital care records over a defined time period, typically one year. In Canadian a… Show more

“…Group Health captures data on all services that physicians and other healthcare providers provide at Group Health facilities, as well as claims for covered services from contracted providers or facilities . Aggregated Diagnosis Groups (ADGs), the base components of the Adjusted Clinical Groups (ACG) case‐mix system, were used to identify participants with serious diagnoses, including major injuries, unstable chronic medical or psychosocial conditions, and malignancies . It was hypothesized that newly diagnosed conditions might be related to changes in patient activation; specifically, based on the model, it was thought that even relatively activated persons may not be able to maintain their behaviors in the face of life stress or health crises.…”

“…The Johns Hopkins University ACG system was chosen for this study because it uses readily available administrative data to explain population level health care utilization. The ACG system has demonstrated validity for explaining the health service needs of Canadian populations [12,22], and has been used in Canada to risk adjust capitation payments and profile physician practices [23].…”
