We proposed to compare the accuracy and effectiveness of digital breast tomosynthesis (DBT), plus digital or synthetic mammography, with digital mammography alone in women attending populationbased breast cancer screenings. We performed a systematic review and included controlled studies comparing DBT with digital mammography for breast cancer screening. Search strategies were applied to the MEDLINE, Embase, LILACS, and CENTRAL databases. With moderate quality of evidence, in 1,000 screens, DBT plus digital mammography increased the overall and invasive breast cancer rates by 3 and 2 (RR 1.36, 95% CI 1.18 to 1.58 and RR 1.51, 95% CI 1.27 to 1.79, respectively). DBT plus synthetic mammography increased both overall and invasive breast cancer rates by 2 (RR 1.38, 95% CI 1.24 to 1.54 and RR 1.37, 95% CI 1.22 to 1.55, respectively). DBT did not improve recall, false positive and false negative rates. However due to heterogeneity the quality of evidence was low. For women attending population-based breast cancer screenings, DBT increases rates of overall and invasive breast cancer. There is no evidence with high or moderate quality showing that DBT compared with digital mammography decreases recall rates, as well as false positive and false negative rates.

Digital breast tomosynthesis (DBT) is emerging as the standard of care for breast imaging based on improvements in both screening and diagnostic imaging outcomes. The additional information obtained from the tomosynthesis acquisition decreases the confounding effect of overlapping tissue, allowing for improved lesion detection, characterization, and localization. In addition, the quasi three-dimensional information obtained from the reconstructed DBT data set allows a more efficient imaging work-up than imaging with two-dimensional full-field digital mammography alone. Herein, the authors review the benefits of DBT imaging in screening and diagnostic breast imaging.

Background: Breast edema in inflammatory breast cancer can cause tremendous skin thickening and breast enlargement that attenuates both the xray and ultrasound beams because of the enlarged, dense breast tissue filled with fluid and tumor. Objective: was to evaluate the value of Contrast-enhanced digital mammography in the assessment of breast lesions in the edematous breast. Patients & Methods: A prospective study conducted on 55 female patients presented with primary breast edema. Mammography followed by contrast mammography was done for all cases. Data were analyzed. Statistical tests were performed to check for sensitivity, specificity, accuracy and predictive values for both modalities. Results: Contrast-enhanced digital mammography was more sensitive and specific in the detection of lesions and better assessment of the local extent of the disease in cases with edematous breast changes. It was accurate in the detection of multifocal/multi-centric disease, being a good positive test in the delineation of masses obscured by condensed parenchymal tissue. The calculated sensitivity of digital mammography & Contrast-enhanced digital mammography was 87.5 %, 95.8%, specificity was 55.5%, 88.8%. The PPV and NPV were 91, 97.9 % and 45%, 80 % respectively. Conclusion: Contrast-enhanced digital mammography is a promising and affordable technique in the assessment of suspicious lesions in mammographically dense breast especially in primary edematous breast cases.

Abstract: FFDM and DBT recall rates were not significantly different in a mixed FFDM and DBT breast imaging practice. However, the PPV1 of recalled cases and the cancer detection rate (the primary screening objective) were significantly higher with DBT compared with FFDM.
