Comparing the cost-effectiveness of two- and three-dose schedules of human papillomavirus vaccination: A transmission-dynamic modelling study

Abstract: Two-dose HPV vaccination is likely to be cost-effective if its duration of protection is at least 10 years. A third dose of HPV vaccine is unlikely to be cost-effective if two-dose duration of protection is longer than 30 years. Finally, two-dose girls & boys HPV vaccination is unlikely to be cost-effective unless the cost per dose for boys is substantially lower than the cost for girls.

“…Vaccine safety and efficacy have been confirmed in pre- and post-licensure studies [11,12,14]. Fewer than the currently recommended three doses of HPV vaccines has been proven to be effective [15,16]. A switch from a three- to a two-dose schedule for primary immunization programmes is hence anticipated, especially in younger adolescents aged between 9 and 13 years [10].…”

“…To systematically select them, we did a literature search on the international database PubMed (period 2010-2014; search terms 'HPV vaccine', 'cost-effectiveness' and 'two doses' or 'nonavalent'). We found three articles [13,18,19] [14] Laprise (2014) [19] Drolet (2013) [20] Journal ( and screened them to assess the main features we considered interesting from our viewpoint (TABLE 1).…”

“…Two compared the twoand three-dose schedules in Canada [18] and the UK [13], the third the nonavalent to the quadrivalent still in the first setting [19]. Two authors co-signed all three studies and four two of them.…”

“…However, the prices of the vaccines vary significantly by region, from as little as $5 to as much as $187 per dose [12]. Recent studies suggest that two doses of vaccine may provide protective immunity [13,14] and be more cost-effective than the three-dose series [15]. However, cost remains the biggest barrier to national implementation of the HPV vaccine in LMICs [16].…”
