Comparing the Safety and Efficacy of Phacogoniosynechialysis With Phacotrabeculectomy in the Management of Refractory Acute Primary Closure Angle Glaucoma With Cataract: A Multicenter Randomized Trial

Abstract: Combined phacoemulsification-goniosynechialysis (phaco-GSL) and unaugmented phacotrabeculectomy were both found to be effective in treating eyes with significant cataract and medically unresponsive acute primary angle closure glaucoma (PACG). Phaco-GSL seemed to be safer, with fewer surgical complications, and achieved better visual acuity than phacotrabeculectomy. Objectives: To compare the results of combined phaco-GSL with unaugmented phacotrabeculectomy in the management of eyes with medically unresponsive … Show more

“…By stripping the PAS from the angle wall, Phaco-GSL aims to restore the normal physiological function of aqueous outflow. However, recent meta-analyses and multicenter studies have produced inconsistent results [ 16 19 ]. We believe that this inconsistency may be attributed to various factors, such as differences in surgical procedures and perioperative management, which must not be overlooked.…”

“…204 A recent RCT compared favourably phaco-GSL to phacotrabeculectomy. 210 GSL physically opens the angle and break PAS, however, it is an inflammatory procedure and PAS can reform. Phaco alone improves IOP and there is no to moderate evidence that GSL adds long-term benefit in PAC and PACG.…”

“…Filtering surgery may be required in eyes with AACC refractory to more conservative measures but carries an increased risk of complications, including malignant glaucoma and choroidal effusions. Nguyen Xuan et al 34 randomized 79 phakic Vietnamese patients with refractory AACC to undergo combined phacoemulsification-GSL or phacoemulsification-trabeculectomy. Intraoperative antifibrotics were not used during phacoemulsification-trabeculectomy, but 1 patient underwent needling with 5-fluorouracil postoperatively.…”
