Comparison between Briganti, Partin and MSKCC tools in predicting positive lymph nodes in prostate cancer: a systematic review and meta-analysis

Abstract: The accuracy of Briganti, Partin and MSKCC was similar in predicting the presence of LNI. Based on these results, patients and clinicians may use any of these nomograms without significant advantages.

“…The MSKCC nomogram showed an AUC of 0.77 similar to the literature (0.78) in a recent meta-analysis (25). With an AUC of 0.67 in our analysis, the updated Partin tables were slightly less accurate than the AUCs in prior cohorts (0.77 and 0.80) (25)(26)(27). The reduced accuracy might be a result of our patient cohort including only high-risk patients, as Partin tables are most powerful in low-and intermediate-risk PC (5,26).…”

“…If lymph node biopsy is indicated according to various nomograms, absolutely lymph node dissection should be performed so that the patient can be correctly evaluated and multi-modal treatment can be performed in case of need. [7] The Tugcu Bakırkoy technique allows for dissection of the pelvic lymph node with less morbidity than other techniques and with superior cosmetic, and equivalent oncological results relative to other methods. This technique does not affect the intestines and there is no need to intervene intraabdominal adhesions developed due to previous surgery.…”

“…This lack of evidence, combined with the perceived increased morbidity of the added procedure, is driving a decline in the practice of lymphadenectomy, except in the presence of high-grade or high-volume disease. Current guidelines recommend treatment of the lymphatic drainage at the time of radiotherapy or radical prostatectomy if the risk of nodal metastasis exceeds 5% by commonly used nomograms such as the Briganti, Partin, and Memorial Sloan Kettering Cancer Center nomograms (usually representing intermediate-and highrisk PC) (67)(68)(69). Guidelines also recommend that primary PC patients with a higher risk profile should not be spared ePLND irrespective of negative findings on imaging (20).…”
