Comparison between Focus Harmonic scalpel and other hemostatic techniques in open thyroidectomy: A systematic review and meta‐analysis

Abstract: According to our meta-analysis, when comparing the Focus Harmonic scalpel versus conventional techniques, it seems evident that the use of this device in thyroid surgery is associated with a mean reduction in operating time, blood loss, and hospital stay, without any increase in pain and complications. There was no appreciable difference between the Focus Harmonic scalpel and the LigaSure Precise Vessel Sealing System. © 2016 Wiley Periodicals, Inc. Head Neck 38: First-1578, 2016.

“…This systematic review updates the data available in the literature with published clinical trials up to the mid-2017. The clinical data found in this study are in line with the latest published reviews on the subject [51][52][53][54] and show that there is apparently no doubt about the actual efficacy and safety of the harmonic scalpel, questioning only the cost-effectiveness of the implementation of this technology in thyroid surgery.…”

“…The blade of the HF vibrates at 55 kHz, producing mechanical energy that breaks hydrogen bonds. The HF is hand-activated, with cutting and sealing of vessels achieved by placing the curved blade in contact with the tissue and applying pressure (3).…”

“…Energy‐based surgical devices (EBD) have been increasingly applied in thyroid surgery due to attractive combined dissection and hemostatic properties. Two instruments that have gained increasing acceptance over the past decade for use in thyroidectomy are the electrothermal bipolar vessel sealing system, or LigaSure (LF1212 Europe, LF1212A USA; Medtronic, Covidien products, Minneapolis, MN), and the Harmonic scalpel (Ethicon, Cincinnati, OH) . These two instruments have been described in terms of reduction of procedure time, incision length, operative blood loss, postoperative pain, hospital stay, and usefulness in endoscopic procedures …”
