Comparison of Free Muscle and Perforator Skin Flaps for Soft Tissue Reconstruction of the Foot and Ankle

Abstract: Free ALT flap consisting of skin and adaptable subcutaneous tissue, both diminishes donor site morbidity and is ideally suited for most soft-tissue reconstruction of the dorsal foot, heel and plantar foot. Free muscle flaps, however, may offer relatively less complicated tissue transfers and are preferred at the ankle region in the presence of open tibia fractures, and in high risk patients to decrease the perioperative morbidity.

“…The processes of identification, screening, and eligibility were described. A total of 1,802 references of free flap were identified and we included 36 studies that discussed 1,226 free flaps used to treat 1,226 patients (Table ). For the pedicled‐propeller flap, a total of 616 references was identified and we included 19 studies .…”

“…In nonobese patients, the ALT flap is well suited for this. 18,19 In western countries, however, the ALT flap is often considerably thicker, 2 limiting its use. Although flap thinning is possible, doing so will not create as thin and pliable a flap as a radial forearm flap, for instance.…”

“…15 Most studies of outcomes of lower extremity reconstruction with the ALT flap have focused on its use as a free flap for relatively distal defects. [16][17][18][19] The ALT flap has also been used as a pedicled flap for repair of more proximal lower extremity defects, though studies of this use often included patients with defects of the abdominal wall, perineum, and back. [20][21][22] In this study, we review our experience with the use of the ALT flap, in pedicled and free forms, for the repair of defects ranging from the hip to the foot.…”
