Purpose To propose a new method of identifying clusters in multifocal electrophysiology (multifocal electroretinogram: mfERG; multifocal visual-evoked potential: mfVEP) that conserve the maximum capacity to discriminate between patients and control subjects. Methods The theoretical framework proposed creates arbitrary N-size clusters of sectors. The capacity to discriminate between patients and control subjects is assessed by analysing the area under the receiver operator characteristic curve (AUC). As proof of concept, the method is validated using mfERG recordings taken from both eyes of control subjects (n = 6) and from patients with multiple sclerosis (n = 15). ResultsConsidering the amplitude of wave P1 as the analysis parameter, the maximum value of AUC = 0.7042 is obtained with N = 9 sectors. Taking into account the AUC of the amplitudes and latencies of waves N1 and P1, the maximum value of the AUC = 0.6917 with N = 8 clustered sectors. The greatest discriminant capacity is obtained by analysing the latency of wave P1: AUC = 0.8854 with a cluster of N = 12 sectors. Conclusion This paper demonstrates the effectiveness of a method able to determine the arbitrary clustering of multifocal responses that possesses the greatest capacity to discriminate between control subjects and patients when applied to the visual field of mfERG or mfVEP recordings. Foi calculada a média das respostas do PEV-mf global, do PEV-mf central e a média de espessura do TCO-dF da mácula para cada quadrante e para cada hemicampo, enquanto a espessura da CFNR foi dividida em 12 setores ao redor do nervo óptico. A perda de CV foi calculada para os quatro quadrantes e para os hemicampos temporal e nasal no CV 24-2 e CV 10-2. Os dois grupos foram comparados utilizando equações de estimativas generalizadas (GEE) e as correlações entre o PEV-mf, CV e o TCO-dF foram calculadas. RESULTADOS: As médias das amplitude P1 e N2 do PEV-mf global e central para os hemicampos e os quadrantes temporais foram significativamente menores nos pacientes que nos controles (p< 0.004). Não houve diferença estatística entre os grupos para os parâmetros de amplitudes do PEV-mf nos setores nasais. Não houve diferença estatística nas médias das latências do PEV-mf global e central entre os pacientes e os controles normais. Foi encontrada correlação moderada, estatisticamente significativa, entre os parâmetros de amplitudes temporais do PEV-mf -global e central com a perda de CV 24-2 e 10-2 temporal, assim como com as medidas de espessura macular e da espessura CFNR na TCO-dF. CONCLUSÕES: As médias das amplitudes do PEV-mf foram capazes de diferenciar olhos de pacientes com hemianopsia dos controles normais e apresentaram correlação significativa com os dados obtidos pela perimetria automatizada e pelo TCOdF. Background and methodsBased on the available literature, it is suggested, in the clinical evaluation of the chiasmal tumors, that the following electrophysiological tests: visual evoked potentials to pattern-reversal stimulation, multifocal visual evoked potentials (mfVEPs), and pattern electroretinogram (PERG) play an important role in the diagnosis of the optic nerve and retinal dysfunction in the course of pituitary tumors.ResultsMacroadenomas and also microadenomas may cause dysfunction of retinal ganglion cells (RGCs) and their axons, even in the absence of changes in the routine ophthalmological examination, retinal sensitivity in standard automated perimetry, and retinal nerve fiber layer thickness in optical coherent tomography. The most frequently observed changes in electrophysiological tests were as follows: in PVEPs—the crossed/uncrossed asymmetry distribution, altered waveform, increase in P100-wave peak time, and/or reduction in amplitude; in mfVEPs—the peak time prolongation and/or amplitude reduction in C1-wave; in PERG—the reduction in N95-wave amplitude and decreased N95:P50 amplitude ratio. Hemifield PVEPs were more often abnormal than full-field PVEPs. Multi-channel recording is recommended for the assessment of the anterior visual pathway. The use of mfVEP offers the possibility to register localized disturbances of the optic nerve and ganglion cells. Additionally, an amplitude of N95-wave reduction in PERG correlated with a lack of postoperative visual acuity recovery. The postoperative improvement in the visual field was found to be associated with a normal N95:P50 amplitude ratio. The RGCs dysfunction manifested by decrease in PhNR/b-wave amplitude ratio was associated with the worse visual fields outcome. A review of the literature summarizing the electrophysiological testing in the pituitary adenoma is discussed.ConclusionIn patients with pituitary tumor, detection of the early dysfunction of the visual pathway may lead to modification of the medical treatment regimen and reduce the incidence of irreversible optic nerve damage.

Abstract: mfVEP, SAP, and OCT provided complementary information for detecting visual pathway abnormalities in patients with PAs. Good agreement was demonstrated between SAP and mfVEP and quantitative analysis of structure-function measurements revealed a moderate correlation.
