Comparison of Sinus Computed Tomography Staging Systems

Abstract: In an attempt to establish a standardized rating system for CT of the paranasal sinuses, the Committee on Rhinology and Paranasal Sinus Disease of the American Academy of Otolaryngology-Head and Neck Surgery instituted a protocol for the review of sinus CT scans at six international sites. Fifty identical scans were rated by four otolaryngologists at each site according to five established sinus CT staging systems. Twenty of 24 reviewers repeated the rating session at least 1 week later to determine intrarater… Show more

“…Several CT staging systems for planning medical and surgical treatment as well as to monitor the therapeutic outcome in CRS have been proposed and evaluated [98,99]. The modified Lund-Mackay system [100] from 1993 has proved to have the highest level of both inter-and intraobserver agreement compared to other scoring systems [98].…”

“…These scans were classified as stage 0 through IV according to a CT rating system that has been used previously and demonstrated to be statistically reliable. 12 The following information describes the CT staging system for chronic rhinosinusitis: Surgical indications for endoscopic frontal sinusotomy include CT evidence of frontal rhinosinusitis (opacification, air-fluid level, mucocele, or mucosal thickening Ͼ2 mm) or recurrent frontal headaches in the presence of radiological or endoscopic evidence of paranasal sinus disease.…”
