Compelling Indications Should be Listed for Individual Beta-Blockers (Due to Diversity), Not for the Whole Class

Abstract: Introduction: Beta blockers (BBs) are an important class of drugs, with numerous indications in cardiology, emergency and general medicine. The indications include heart failure (HF), acute myocardial infarction (AMI), post-MI [1], supraventricular and ventricular arrhythmias, chronic coronary syndrome, systemic arterial hypertension (HTN), hypertrophic cardiomyopathy and aortic aneurysm or dissection. All BBs decrease heart rate (HR) and blood pressure (BP), the major determinants of myocardial oxygen consum… Show more

“…For example, ivabradine attenuates heart failure by reducing the heart rate of patients [ 33 ]. In addition, β -blockers, as commonly prescribed drugs for the treatment of cardiovascular diseases, decrease heart rate to limit myocardial oxygen consumption, thereby improving heart function [ 34 ]. Some traditional Chinese medicines, such as ginsenoside, one of the active ingredients of ginseng, reduce heart rate, thereby alleviating cardiac dysfunction and remodeling in heart failure [ 35 ].…”

“…In CS/CD patients with HTN and "borderline" HF (incipient symptoms with minimal NT pro-BNP elevation): in the first line diuretic (MRA), RAAS blocker, and certain BBs (bisoprolol, carvedilol, metoprolol succinate [50] and nebivolol [25]) can be recommended. An adequate choice of certain BBs is needed because they are a very different class of drugs [51]. Also SGLT2 inhibitors can be suggested for evaluation in CS/CD patients.…”

“…β -Blockers differ a lot in their pharmacological properties, for instance receptor subtype specificity, intrinsic sympathomimetic activity, membrane-stabilizing activity, lipophilicity, side effects and effectiveness in treating heart failure. Thus, they need to be studied individually rather than by category [125].…”
