Pneumosinus dilatans (PSD) is a rare disorder that causes aberrant paranasal sinus dilation. Pneumosinus dilatans is typically benign and asymptomatic, without any evidence of bone destruction or pathologic changes in the underlying mucosa. Localized discomfort, headaches, facial paraesthesia and deformities, sensory deficits, olfactory loss, and ocular abnormalities are common complaints. The diagnosis of PSD is confirmed by the radiological investigations. The surgical treatment’s objective is to return both form and function of the affected sinus to normal. We briefly present our experience with pneumosinus dilatans and discuss the characteristics of this disease based on the specialised literature.

Цель работы – продемонстрировать рентгенологические диагностические критерии гиперпневматизации околоносовых пазух и височной кости. Данные конусно-лучевой компьютерной томографии (КЛКТ) двух пациентов с амбулаторного лор-приема, полученные на аппарате ProMax®3D Mid (Planmeca Oy, Helsinki, Finland), были оценены в программе Planmeca Romexis. Степень пневматизации оценивалась с использованием комплекса рентгенологических параметров, включая распространенность воздухоносной полости в пределах границ соответствующей кости, а также ее протяженность относительно соседних анатомических структур. В литературе описано более 50 вариантов строения воздухоносных костей черепа, включая их гиперпневматизацию (Earwaker, 1993; Marino et al., 2020). Диагностирование гиперпневматизации сопряжено с определенными сложностями, так как у взрослого населения без видимой/верифицированной патологии линейные размеры и объемы лобной, верхнечелюстной и клиновидной пазух варьируют в широком диапазоне (Hettiarachchi et al., 2021; Metin-Gürsoy et al., 2020; Jaworek-Troć et al., 2021). Поэтому, кроме оценки размеров воздухоносной полости, следует учитывать и другие критерии. Описанные нами случаи характеризуются гиперпневматизацией одновременно нескольких воздухоносных костей черепа в сочетании с другими индивидуальными вариантами их строения. Полученная информация, в частности о взаимоотношениях увеличенных в размерах клиновидных пазух и ячеек решетчатой кости с внутренней сонной артерией и черепными нервами, представляет интерес для рентгенологов, оториноларингологов и челюстно-лицевых хирургов. The purpose of the study was to demonstrate radiologic diagnostic findings of the paranasal sinuses and temporal bone hyperpneumatization. Сone-beam computed tomography (CBCT) data from two outpatient patients, obtained with the ProMax®3D Mid digital dental X-ray machine (Planmeca Oy, Helsinki, Finland), were evaluated in the Planmeca Romexis program. The degree of skull bones pneumatization was assessed using a set of parameters, including the extent of the air cavity within the boundaries of the corresponding bone, as well as its expansion relating to adjacent anatomical structures. More than 50 variants of the structure of the pneumatized skull bones, including their hyperpneumatization, have been described in the literature (Earwaker, 1993; Marino et al., 2020). Diagnosing hyperpneumatization is associated with certain difficulties, because in the adult population without visible/verified pathology, the linear dimensions and volume of the frontal, maxillary and sphenoid sinuses vary considerably (Hettiarachchi et al., 2021; Metin-Gürsoy et al., 2020; Jaworek-Troć et al., 2021). Therefore, in addition to assessing the size of the air space, other criteria should be considered. The described cases are characterized by simultaneous hyperpneumatization of several pneumatized skull bones in combination with other individual variants of their structure. The information obtained on the relationship of enlarged sphenoid sinuses and ethmoidal air cells with the internal carotid artery and cranial nerves, is of particular interest to radiologists and endoscopic surgeons.

Pneumosinus dilatans (PD) is a rare clinical entity involving expansion of the paranasal sinuses. While the pathogenesis of PD is not fully understood, the prevailing theory involves a “ball-valve” mechanism in which obstruction of sinus outflow leads to progressive air cell expansion. Typically, the progression of PD can be halted with endoscopic sinus surgery to relieve any outflow obstruction. In this report, the authors present a case of a patient with progressive expansion of the frontal sinus refractory to endoscopic sinus surgery. The patient ultimately required excision of the anterior table of the frontal sinus and reconstruction with a custom polyetheretherketone implant. This case highlights the variability of PD presentation and how treatment and reconstruction must be tailored to each patient’s unique presentation.
