Comprehensive review of the diagnosis and treatment of biliary tract cancer 2012. PART II: Multidisciplinary management

Abstract: Biliary tract cancers (gallbladder cancer, intra- and extra-hepatic cholangiocarcinoma and selected periampullary cancers) accounted for 12,760 new cases of cancer in the USA in 2010. These tumors have a dismal prognosis with most patients presenting with advanced disease. Early, accurate diagnosis is essential, both for potential cure where possible and for optimal palliative therapy in all others. This review examines the currently available and emerging technologies for diagnosis and treatment of this group… Show more

“…The morbidity and mortality of surgical intervention to establish drainage have largely mitigated against this approach in the face of increasing evidence of the efficacy of biliary stenting. There has, however, been extensive investigation as to the optimum use of stents to palliate biliary obstruction associated with cholangiocarcinoma [2,3].…”

“…Worldwide, cholangiocarcinoma is the second most common primary liver cancer, after hepatocellular carcinoma (HCC) [1][2][3][4]. In the United Kingdom, cholangiocarcinoma kills approximately 1500 people per year and since the 1990s, more deaths have been coded to cholangiocarcinoma than to HCC [5,6].…”

“…Surgical resection, combination chemotherapy and radiotherapy have been acknowledged to improve patient prognosis as the multimodal treatment for advanced cancer (1,2). However, overall survival has remained low in advanced cancer and is a challenging obstruction to overcome (3).…”

“…3 Metal stents (bare metal mesh) are usually preferred and carry the advantage of longer duration of patency compared with plastic stents. 4 There is controversy over unilateral versus bilateral stents in unresectable hilar biliary obstruction, as only a portion of the liver will be drained with a single stent. 2 Tumor ablation combined with stenting can reduce cholestasis and improve median survival time in patients with cholangiocarcinoma.…”
