Computed tomography and magnetic resonance imaging evaluation of pelvic lymph node metastasis in bladder cancer

Abstract: BackgroundAccurate evaluation of lymph node metastasis in bladder cancer (BCa) is important for disease staging, treatment selection, and prognosis prediction. In this study, we aimed to evaluate the diagnostic accuracy of computed tomography (CT) and magnetic resonance imaging (MRI) for metastatic lymph nodes in BCa and establish criteria of imaging diagnosis.MethodsWe retrospectively assessed the imaging characteristics of 191 BCa patients who underwent radical cystectomy. The data regarding size, shape, den… Show more

“…In our study 6 (20%) lymph nodes were round in shape and 6 (20%) lymph node with low attenuated hilum that indicated malignancy features by CT which is consistent with Yong Li, Feiju Diao, Siya Shi [22] reported 5 (16%) oval lymph nodes and 4 (12%) lymph node with low attenuated hilum.…”

“…As shown in a study by Mir et al ( 57 ), a fusion of high b-value (750 s/mm 2 ) DW-MRI with conventional T2-weighted images could improve lymph node identification. In the study by Li et al ( 22 ) the presence of fatty hilum was indicative of non-metastatic character of a lymph node in bladder cancer patients. Papalia et al ( 40 ) have stated that DW imaging sequences could increase the sensitivity of MR in detecting lymph node metastases to 76%, with the optimal ADC cut-off value of 0.86×10 −3 mm 3 /s.…”

“…Apart from the short axis size, the shape of a lymph node may be revealing of its status, with a round shape indicative of metastasis and an oval shape characteristic of an inflammatory response. Hence, the long-to-short axis ratio has been proposed as a measurable indicator of LNI ( 22 ). Yet, this parameter has not been shown to improve the diagnostic performance compared to short axis diameter alone ( 22 ).…”
