Computed tomography versus ultrasound of the adrenal gland: a prospective study.

Abstract: A prospective study of the accuracy of computed tomography (CT) and ultrasound was undertaken in 112 consecutive patients with suspected adrenal disease. CT had a sensitivity of 84% (47/56), a specificity of 98% (55/56), and an accuracy of 90%. Ultrasound had a sensitivity of 79% (22/28), a specificity of 61% (14/23), and an overall accuracy of 70%. When patients with Cushing disease and adrenal hyperplasia were excluded, and only masses such as pheochromocytoma, adenoma, and carcinoma were considered, the sen… Show more

“…Obesity and overlying gas are frequent obstacles for visualization of the adrenal glands (25). Thus, US does not detect adrenal masses with the same sensitivity as CT or MRI (26,27). According to one study (28), the sensitivity in detecting incidentalomas depends on the mass size, being 65% for lesions !3 cm and 100% for lesions O3 cm.…”

“…7 Computed tomography is the preferred method for evaluating adrenal gland morphology 9 and has been extremely useful in detecting the adrenal abnormalities associated with this disease. 7 Abdominal sonography commonly has heen used as a screening modality for patients with nonspecific complaints such as weight loss and chronic fever.…”

“…Th e AIs are detected by CT scan at about 28% (Mantero et al 2000). CT is the primary diagnostic imaging modality for the evaluation of the adrenal disorders (Karstaedt et al 1978;Montagne et al 1978;Wilms et al 1979;Abrams et al 1982). On the pre-contrast CT, the adrenals have a soft tissue density similar to that of the liver .…”

“…Large and small masses and hyperplasia can be readily detected when present . With proper technique, masses smaller than 5 mm can be detected (Korobkin et al 1979;Eghrari et al 1980;Abrams et al 1982). Th e accuracy of CT for diagnosis of adrenal masses has been reported as being better than 90% ) and a normal appearance of the adrenal eff ectively excludes the presence of an adrenal tumor .…”
