Conservative management for patients with chronic kidney disease refusing dialysis

Abstract: Estimates suggest that 20-30% of the deaths of patients with chronic kidney disease with indication to undergo dialysis occur after refusal to continue dialysis, discontinuation of dialysis or inability to offer dialysis on account of local conditions. Contributing factors include aging, increased comorbidity associated with chronic kidney disease, and socioeconomic status. In several occasions nephrologists will intervene, but at times general practitioners or family physicians are on their own. Knowledge of … Show more

“…The glomerular filtration rate determines the stage of chronic kidney disease and its control affects the course of treatment, since it aims to assess renal function from the clearance of substances that are freely filtered by the glomerulus and do not undergo reabsorption or secretion tubular. Thus, it is shown as an important indicator in conservative treatment and needs to be determined by a specialized health professional who directs the best strategies (41) .…”

“…It is important to have an articulated and complementary relationship "between the different professions, since knowledge construction and best care practices" (21) require national and international interdisciplinary and multidisciplinary support networks in order to humanize and fully meet the needs of the person in a dialogical perspective (22)(23) . However, the confrontations imposed on nurses in the context of the nephrology service do not refer to the patient itself, but to complex interpersonal relationships, which may affect all therapy, even compromising the quality of the care provided (24) .…”

“…Approaches to managing diabetes vary. Individuals previously on insulin therapy are often required to decrease dosages, since insulin is metabolized in the renal tubular cells and has its half-life prolonged in CKD G5 [153]. A recent study, among a cohort of US veterans, shows the need for different glycemic strategies based on whether there are plans to transition to dialysis versus pursuing comprehensive conservative care among DKD patients by relating mortality to different glycated hemoglobin targets in each of the groups [154].…”
