Continent cutaneous diversion

Abstract: Continent cutaneous diversion is rarely offered in the USA to patients undergoing cystectomy except in a few centers. Most studies have found a high complication rate and need for revision surgery in 10-20% of patients. However, functional results are acceptable and many patients are willing to accept the complications in exchange for avoiding an external appliance.

“…Continent cutaneous diversion is rarely performed today in conjunction with RC . In patients undergoing RC in Sweden for primary bladder cancer, 19% received such a diversion in 1997, while this figure was only 1% in 2014 (The Swedish Quality Registry for Bladder Cancer 2014) .…”

“…1 A right colon pouch urinary diversion relies upon de-tubularization of the cecum and ascending colon to create a spherical low-pressure reservoir or pouch. 2 A continent cutaneous catheterizable channel is also created, most often utilizing a segment of terminal ileum or appendix, which a patient catheterizes as the "efferent channel" to empty the pouch. This procedure is highly complex and has been suggested to have a higher rate of complications compared to the ileal conduit.…”
