In Spain, approximately 10 years passed between the time when human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) harm-reduction programs should have been developed with sufficient coverage to have an optimum impact on public health (before the HIV/AIDS epidemic's explosion in 1984) and the date of their actual implementation. This delay yielded an enormous cost for the country. The introduction of the virus in drug injector networks during a period of widespread diffusion of heroin injection and the lack of political awareness of the growing problem were 2 important factors that contributed to the important diffusion of the HIV infection among Spanish injection drug users. Lessons can be learned that may be of great interest in countries or territories facing similar challenges now and in the future.

Spain is the European country with the highest cumulative AIDS caseload and presently has an estimated population of at least 120,000 persons living with HIV. Most Spanish HIV and AIDS cases are due to needle sharing in the context of intravenous drug use. This article traces the development of targeted HIV prevention policy in the ®eld of drug use, exploring the reasons for the time-lag in making harm reduction among drug users a policy priority. Reports about dramatically high HIV prevalence rates among Spanish drug users had already emerged by 1986±1987. However, the Spanish government began to devise large-scale harm reduction programmes for drug users only in the years 1992±1994. This new development has led to the establishment of Europe's largest methadone maintenance programme. In the meantime, though, the central government (as opposed to health authorities in some of Spain's regions) continued to treat public health concerns as secondary to a drug policy focused on the goal of abstinence. The article argues that both (a) the rapid diffusion of HIV among drug users in the late 1970s and early 1980s and (b) the time-lag in elaborating an adequate policy reaction to the emerging AIDS crisis were related to the country's transition from an authoritarian regime to democratic government. Rather than in terms of direct continuity, it is argued, authoritarianism left its mark on HIV epidemiology and policy in terms of the fast social change triggered by its demise.

RESUMENUno de los objetivos esenciales de la Bioética es incorporar los valores, junto a los hechos, en las decisiones propias de aquellas disciplinas científicas relativas a la Vida. En ámbi-tos asistenciales la Bioética pretende desvelar y superar aquellos conflictos de valores derivados de una interrelación en la que se implican sujetos individuales y / o grupos sociales.Las drogodependencias representan un campo idóneo para este análisis porque suscitan conflictos de valores a niveles muy diversos. Constituyen un problema complejo en el que interaccionan intereses políticos, económicos, sociales, culturales y sanitarios.En el ámbito asistencial, la reflexión bioética, tanto en su momento deontológico como teleológico, aporta una perspectiva complementaria a la terapéutica. Puede facilitar el abordaje de numerosos problemas, que van de la identificación del marco conceptual a la detección de los criterios de elección entre distintos programas (libres de drogas, mantenimiento o reducción de daños).Un acercamiento progresivo a aspectos como delimitación y protocolización de buenas prácticas clínicas, valoración de la competencia, desarrollo de la autonomía individual, equidad en la distribución de recursos asistenciales o implementación de medida de beneficencia, entre otros aspectos, puede ayudar a comprender, integrar y superar algunos problemas de las drogodependencias desde una perspectiva asistencial.
