Corneal profile analysis after Descemet stripping endothelial keratoplasty and its relationship to postoperative hyperopic shift

Abstract: The donor graft underwent changes after DSEK, which may account for the induced hyperopia and its diminishment over time via changes in the posterior corneal curvature. Results suggest that intraocular lenses be targeted to -1.00 to -1.25 D of myopia for combined DSEK and cataract procedures.

“…On the other hand, such an approach can yield to error due to the variability in amount of hyperopic shift between patients: donor lenticule parameters have been hypothesized to be the main factors of postoperative refractive inaccuracy [11][12][13][14][15].…”

“…EK is known to result in hyperopic shift owing to increased thickness and curvature of the posterior cornea. 56 LOAs are easily recognised and are amenable to refractive correction and are therefore less problematic than other determinants of vision. Accordingly, we concentrate on those factors that cannot be readily diagnosed or rectified.…”

“…18,41,56,141 Reduction in posterior corneal curvature over time leads to diminishing hyperopia. 56 Hyperopia is increased by grafts with thick peripheries and thinner central regions acting as minus meniscus lenses. 8,141 Newer techniques with thinner grafts have produced less hyperopic shift, in the order of 0.75D.…”
