Coronary Artery Spasm Demonstrated by Coronary Angiography in a Patient with Acute Myocarditis Resembling Acute Myocardial Infarction; A Case Report.

Abstract: A 59-year-old male with acute myocarditis presenting symptoms resembling acute myocardial infarction underwent urgent coronary angiography due to difficulties in determining a diagnosis. Coronary artery spasm was demonstrated in the proximal right and left anterior descending coronary arteries. The spasm varied from time to time and lesion to lesion. But no fixed occluded lesion was observed. Predischarge coronary angiography showed no abnormal findings. Histological findings of the biopsy specimen from the le… Show more

“…In our series 21 of the 100 patients underwent coronary angiography or cardiac CT, all had normal or non-significant coronary artery disease. Coronary spasm in myocarditis has been previously proposed [9-11], but cases of true coronary spasm proven on coronary angiography in patients with acute myocarditis/pericarditis are rare [12,13]. In a recent study [14] biopsy proven PVB19 myocarditis was strongly associated with coronary spasm induced by acetylcholine testing during coronary angiography.…”

“…Myocarditis has been shown to cause variant angina and mimic AMI [4,5] and is a plausible cause in this case as our patient experienced a viral prodrome prior to his onset of pain.…”

“…There are several mechanisms suggested that vasoactive substances (e.g. thromboxane A2), myocarditis-induced endothelial dysfunction, smooth muscle hyperactivity may induce coronary artery spasm in acute myocarditis patients [ 12 – 14 ].…”
