Coronary Plaque Classification With Intravascular Ultrasound Radiofrequency Data Analysis

Abstract: Background-Atherosclerotic plaque stability is related to histological composition. However, current diagnostic tools do not allow adequate in vivo identification and characterization of plaques. Spectral analysis of backscattered intravascular ultrasound (IVUS) data has potential for real-time in vivo plaque classification. Methods and Results-Eighty-eight plaques from 51 left anterior descending coronary arteries were imaged ex vivo at physiological pressure with the use of 30-MHz IVUS transducers. After IVU… Show more

“…Advanced mathematical algorithms including radiofrequency analysis (RFA) and elastography allow a more detailed analysis 96,97 . IVUS-derived RFA (virtual histology, VH-IVUS) displays the reconstructed data as a color coded tissue map of plaque composition based on validation to histology [98][99][100] . VH-IVUS classifies plaque components into four basic tissue types: fibrous (dark-green), fibro-fatty (light-green), necrotic core (red), and dense calcium (white) 101 .…”

“…Recently, spectral analysis of IVUS radiofrequency data (Virtual Histology IVUS [VH-IVUS]) has demonstrated potential to differentiate 4 components of atherosclerotic plaques (ie, fibrous, fibro-fatty, dense calcium, and necrotic core). 4) Because the axial resolution of IVUS is between 100 to 150 μm, several VH-IVUS studies have proposed that the absence of visible fibrous tissue overlying a necrotic core indicated a cap thickness of less than 100 to 150 μm and used the absence of such tissue to identify TCFA. 5) In the present study, we evaluated the diagnostic accuracy of VH-IVUS to identify TCFA compared with OCT.…”

“…16,11) In vivo studies demonstrate that hs-CRP was associated with positive remodelling, fibrous cap thickness, and the presence of TCFA. 17,18) Furthermore, a higher prevalence of lesions with rupture ultrasonographic changes, with positive remodelling or lipid lakes was found in individuals with high CRP levels, as compared to patients with normal levels of this marker.…”
