Could aquaporin modulators be employed as prospective drugs for COVID-19 related pulmonary comorbidity?

Abstract: COVID-19 initially an epidemic caused by SARS-CoV-2 has turned out to be a life- threatening global pandemic with increased morbidity and mortality. The presence of cytokine storm has been linked with the pathogenesis of severe lung injury as evinced in COVID-19. Aquaporins (AQPs) are molecular water channels, facilitating water transport across the cell membrane in response to osmotic gradients. Impairment in alveolar fluid clearance due to altered functional expression of respiratory AQPs highlight their pat… Show more

“…The role of aquaporins (AQPs) water channels across cell membranes is also important, particularly in lungs, with their isoforms AQP1, AQP3, AQP4 AQP5. 44 All these aspects have a clear clinical value in COVID-19.…”

“…We may also consider the role of RL‐PBMT in improving water transport and catabolic evacuation by releasing bound water, facilitating alveolar fluid clearance in pulmonary oedema, hence supporting self‐repair mechanisms and faster return to homeostasis. All these aspects have a clear clinical value in COVID‐19.…”

“…Some of the aquaporins, including AQP3, essentially function as water transport pores between the airways and the pulmonary capillaries [40], are key in lung fluid clearance and the formation of this lung oedema as a consequence of the lung injury [36]. In fact, the use of aquaporin modulators in lung inflammation and oedema has been proposed for potential use for the treatment of COVID-19 respiratory comorbidity [41].…”

“…All these phenomena collectively may lead to increased pulmonary edema or pulmonary hypertension following SARS-CoV-2 infection. Pulmonary edema rapidly developing in COVID-19 patients with COPD is considered as one of the reasons for death ( Mariajoseph-Antony et al, 2020 ). Transporter proteins such aquaporins, which are in the check point of cellular inflammations and pulmonary edema, may be considered as potential drug targets to modulate their expression levels to mitigate the inflammation-induced comorbidity in COVID-19 patients ( Mariajoseph-Antony et al, 2020 ; Azad et al, 2021 ).…”

Moreover, a medication strategy of COPD patients had also been claimed to be associated with the risk of COVID-19.…”
