Rationale:Pulmonary spindle cell carcinoma (PSCC) is a highly malignant carcinoma that often exhibits the histopathological characteristic of cell pleomorphism.Patient concerns:Here, we report a PSCC case in a 59-year-old woman with multiple metastases. The pathological manifestation of this case did not show high-grade pleomorphism or atypia, and was easily mimicked by other borderline or low-grade malignant tumors such as an inflammatory myofibroblastic tumor (IMT).Diagnoses:Based on the results of immunohistochemistry and fluorescence in situ hybridization after the operation, IMT was ruled out, and PSCC was confirmed.Interventions:Resection of the right middle lobe and dissection of the lymph nodes were performed. The patient was given 2 cycles of chemotherapy with cisplatin and 1 cycle of radiotherapy after the operation.Outcomes:Metastatic lesions in the left kidney, the right lung, the first thoracic vertebrae, the retroperitoneal lymph nodes, and the mediastinal lymph nodes were identified by positron emission tomography/computed tomography 4 months after the operation, and the patient died 2 months later.Lessons:PSCC is a highly malignant carcinoma, but it rarely shows a low-grade malignant histological morphology. So, efforts should be focused on differentiating it from other borderline or low malignant tumors (such as IMT) to avoid misdiagnosis.

Its clinical features are unclear because of its rarity. Here, we report an autopsy case of SPCC and review CT findings and chemotherapeutic regimens based on previous reports of this disease. To our knowledge, this is the first reported case of pemetrexed used to treat SPCC.Case Report:A 74-year-old Japanese male presented with dyspnea and contrast-enhanced computed tomography (CT) showed abundant left pleural effusion and a mass in lower lobe of the left lung. By the tumor biopsy, he was diagnosed for SPCC of the lung, cT3N0M1a, stage IV. The tumor was resistant to chemotherapy with carboplatin and pemetrexed, and rapidly progressed. Autopsy revealed abundant hemorrhage within the tumor, which apparently reflects a low-density area in CT.Conclusions:Present case and the accumulation of cases indicate that low-density areas in CT and rapid tumor progression may be common SPCC findings.

Spindle-cell lung carcinoma is a very rare disease, one of the forms of biphasic squamous-cell carcinoma, which can be detected in isolated cases. Despite different descriptions of this pathology, currently there is no consensus about methods of its exact morphological diagnosis and effective treatment. Therefore, it is very important to study all existing at the moment results of spindle-cell carcinoma research for the purpose of systematization and analysis of available data and development of effective methods of the disease control. The review contains modern understanding of clinical medicine about the tumor and different methods of its classification and identification based on worldwide scientific experience. The results of the research of spindle-cell lung carcinoma development causes are discussed, and statistical measures of its detection and used treatment methods are described. Spindle-cell lung carcinoma, frequently affecting male smokers as a large peripheral focus, has unfavourable prognosis even in those cases when the diagnosis was made at the early stage and radical surgery was performed. Distant metastases occur more frequently and earlier than in other tumors of the same group, and survival after recurrence is very low. One of the most promising methods of treatment in case of such malignancy is targeted immunotherapy such as anti-PD-1/PD-L1 antibodies aimed at inhibiting immune control point.
