Coxsackie B2 Virus Infection Causing Multiorgan Failure and Cardiogenic Shock in a 42-Year-Old Man

Abstract: Infections from coxsackie B2 viruses often cause viral myocarditis and, only rarely, multisystem organ impairment. We present the unusual case of a 42-year-old man in whom coxsackie B2 virus infection caused multiorgan infection, necessitating distal pancreatectomy, splenectomy, renal dialysis, and venoarterial extracorporeal membrane oxygenation with mechanical ventilation. In addition, the patient had a rapid-eye-movement sleep–related conduction abnormality that caused frequent sinus pauses of longer than 1… Show more

“…CVB infections are found worldwide, and they primarily occur in children and young adults. The course of myocarditis differs between individuals, with outcomes including complete recovery and disease resolution, cardiogenic shock, atrioventricular fibrillations and sudden cardiac arrest, the development of autoimmune myocarditis, and the onset of dilated cardiomyopathy [6,31,32]. The reason for such variation in outcomes is still not fully understood.…”

“…In addition, presence of slow-growing or intracellular microorganisms like Bartonella quintana, Staphylococcus aureus, Streptococcus equi, and Streptococcus oralis may also cause BCNE [4,5]. Helicobacter pylori, Mycoplasma pneumonia, Bacillus typhosus and Chlamydia pneumonia, and viral pathogens including fowl adenovirus serotype 4 (FAdV-4), parvovirus B19, Hepatitis C Virus (HCV), Enterovirus (Coxsackie Virus), thecytomegalovirus and herpes simplex viruses have long been known to cause coronary artery disease, myocarditis and pericarditis [6][7][8][9]. Myocarditis caused by Staphylococcus aureus often results into sepsis (bacteria growing in the blood) and may form abscesses in the heart [6].…”

“…The usefulness of ECMO in patients with acute or fulminant viral myocarditis with hemodynamic instability has been reported in infections caused by enterovirus and Coxsackie virus [11,12]. However, there are no reports showing that ECMO support is successful in treating myocarditis secondary to SFTS.…”
