Creatinine- vs. cystatin C-based equations compared with 99mTcDTPA scintigraphy to assess glomerular filtration rate in chronic kidney disease

Abstract: At GFR <60 ml/min, CKD-EPI and Hoek equations appeared to best correlate with (99m)TcDTPA. In controls and at early stages of CKD, creatinine-based equations correlated better with (99m)Tc-DTPA, with CKD-EPI being the one with the best degree of agreement.

“…Generally, patients with concomitant diseases or chronically taking medications are known to have less reliable GFR measured by DTPA (probably because of alterations in protein-binding of DTPA) [30]. We are also not sure if and how an increased ratio of fat tissue may influence reliability and reproducibility of the DTPA scintigraphic measurement of GFR [31].…”

“…All these formulae lack the proper validation at the GFR at which they were applied because the creatinine methods were not standardized among the intervening centers, giving rise to differences in creatinine measurements [2, 4]. Finally, creatinine-based estimations of GFR present many drawbacks and depend on many variables, and the precision of these equations remains under intense debate [5, 6]. …”

“…Many equations based on creatinine and cystatin C have been developed. In this regard, we have recently published a prospective study in which kidney function was evaluated employing 99m Tc-DTPA as the gold standard of GFR and creatinine or cystatin C equations [6]. In that manuscript in which 300 subjects were included the main conclusions were that at GFR < 60 mL/min, CKD-EPI creatinine and cystatin C-based Hoek equations gave the best correlations with 99m Tc-DTPA.…”

“…In early stage renal failure patients and healthy population, statistical significance was found to be continuing with declining correlation (r ¼ 0.60, P50.001). 30 In a study Huang and colleagues have made in a heterogeneous group of 42 adult patients, they compared Filler, Grubb, Hoek, Larsson, Le Bricon ve Rule formulas with multiple blood sampling method using Tc-99m DTPA. They found that Cystatin-C based formulas showed a quite good correlation among themselves and that correlation coefficients with radionuclidic method changed between 0.87 and 0.65 (each P50.001).…”
