Several genital anomalies have been reported in the identical twins which have a tremendous effect on reproductive status. No previous studies have reported the Mullerian duct cyst in identical twin brothers. We describe a rare case of Mullerian cyst in a male identical twin with infertility. A 43-year-old man presented with 2 years of infertility. In the spermogram analysis, sperm count leaded to azoospermia detection. Transrectal ultrasonography (TRUS) examination was done. An echo-free structure in the mid part of prostate suggested a Mullerian cyst which had caused ejaculatory duct obstruction. The other twin, who dealt with infertility as well, was referred for TRUS. A Mullerian cyst was detected. Ultimately, testicular sperm extraction and percutaneous epididymal sperm aspiration procedures were recommended. Imaging with variety ranges of modality can help to identify Mullerian cyst. Further researches for detecting the genetic factor causes of this anomaly should be considered.

Cyst of Hattori: A Rare Cyst in the Posterior Mediastinum

Sixteen years since their initial description in the literature, posterior mediastinal Mullerian cysts – otherwise known as cysts of Hattori, after their discoverer – remain rare, with only 40 patients reported as of 2020. We report a 43 year old obese female that presented with a 2.6 cm cyst found in the posterior mediastinum by endoscopic ultrasound (EUS) and previously by chest magnetic resonance imaging (MRI) originally diagnosed as a congenital enteric duplication cyst of the esophagus radiologically. Upon surgical excision via a DaVinci thoracoscopy, the cyst was confirmed to be of Mullerian origin by PAX8, WT1, and ER staining. It is possible that the majority of cysts of Hattori remain unrecognized and undiagnosed, given their rarity and resemblance to other pathologies. It is important that this entity become a part of every pathologist's differential for a posterior mediastinal cyst in a female.

A Müllerian cyst in a male adolescent: a case report and literature review

Müllerian duct cyst is a congenital anomaly that originates from an abnormality in the degradation of the Müllerian system and remnants of the Müllerian duct. Müllerian cyst is rarely reported in male adolescents younger than 18 years of age. In this case report, a 17-year-old male adolescent presented with a scrotal mass of 6 years’ duration. Ultrasonography and computed tomography revealed a left scrotal mass, and the patient underwent open surgical resection of the cyst. Histologically, the cyst consisted of tall columnar cells and was consistent with a Müllerian duct cyst. We discuss the presentation and imaging findings in this case, drawing attention to the diagnosis and treatment.
