Cuff width influence on blood pressure measurement during the pregnant‐puerperal cycle

Abstract: Our findings showed that the SCW underestimates BP of pregnant women. Our hypothesis is that such underestimation may lead to the misdiagnosis of pre-eclampsia, particularly in lean pregnant women.

“…BP was measured by our medical staff with a manual mercury sphygmomanometer and an appropriate-sized cuff (length of 1.5 times the circumference of the arm) was used. It is well known that the use of a standard cuff width 50 or of automated oscillometric devices 51 underestimate BP of pregnant women and may affect the correct evaluation of BP during pregnancy.…”

“…In lean women, it was underestimated, on average, 23 mmHg SBP and 20 mmHg DBP. The results indicated BP underestimation when measured with conventional cuff, with greater underestimation in lean women (13) . In the second study, two BP readings were obtained, one quickly and the other with suitable lowering of mercury.…”

“…The primary studies included in this review suggest as recommendations: BP measurement with appropriate cuff for AC (13) ; slow speed deflation of the mercury column (≤2 mmHg) to obtain more reliable and more precise results in the diagnosis (14) , early identification and drug treatment of hypertensive crisis through institutional protocols (15) . The Association of Women's Health, Obstetric and Neonatal Nurses in the United States and Canada -AWHONN released a clinical protocol focused on the diagnosis of hypertensive disorders.…”

“…As diferenças encontradas entre nossos resultados e os de outros autores (16)(17)(18)(19)(20)(21)(22) , podem estar relacionadas às peculiaridades de cada população. Além disso, no atual estudo foram usados manguitos de largura 8, 9, 12 e 15 cm, enquanto os autores citados utilizaram manguitos com largura variando de 1 em 1 cm, a partir de 8 cm.…”
