Current Asthma Guidelines May Not Identify Young Children Who Have Experienced Significant Morbidity

Abstract: The NAEPP guidelines 2002, based on symptom-frequency criteria as assessed in this study, seem to offer a valid basis for classifying asthma severity in those >5 years of age but may underclassify younger children. Our data suggest that morbidity experienced in the prior year may provide a useful additional criterion for classifying asthma severity, particularly in those children < or =5 years of age.

“…There is no gold standard for diagnosing asthma nor for rating the severity of the condition. However, a recent study concludes that the 2002 National Asthma Education and Prevention Program (NAEPP) guidelines, based on symptom-frequency as used in this study, seem to offer a valid basis for classifying asthma severity in those greater than 5 years of age (43). However, this method may underclassify younger children (43).…”

“…Children who are at risk for exacerbations from asthma may not experience greater asthma symptoms before the onset of the severe exacerbation. 9 To our knowledge, no studies have examined both the predictors of asthma symptoms and the predictors of severe exacerbations using longitudinal data in children. Predictors of asthma symptoms in adults include use of oral corticosteroids, comorbid illnesses, higher BMI, and low income.…”
