Current knowledge in the anatomy of the human anterior cruciate ligament

Abstract: The anterior cruciate ligament (ACL) is one of the most frequently studied structures of the musculoskeletal system and continues to stimulate debate and challenges among researchers and surgeons. The ultimate goal of anatomic reconstruction surgery is to restore the native anatomy as much as possible. However, this requires thorough knowledge of its anatomy. The aim of this article is to review the current knowledge of the anatomy of ACL along with its macrostructural and ultrastructural properties.

“…26 The mean width of the ACL calculated in the current study was 8.2 mm, ranging from 5.1-13.7 mm. This measurement differs slightly from the range of 7-12 mm as determined by Bicer et al, 7 Zantop et al 5 and Odensten and Gillquist. 25 Previous authors found the ACL to have a mean width of 10 mm 25 and 11 mm.…”

“…The literature has shown different results for the mean length of the ACL, namely: 20 mm, 24 31 mm, 25 32 mm, 5,7,9 38 mm, 8 and 39 mm. 26 The ACL length has exhibited some variability, with reported ranges of 20-25 mm, 13 22-41 mm, 9 22-44 mm, 7 25-35 mm, 25 and 37-41 mm. 26 The mean width of the ACL calculated in the current study was 8.2 mm, ranging from 5.1-13.7 mm.…”

“…In cross-section, the smallest part of the ligament is at the mid-substance, measuring between 36 and 48.9 mm 2 (Bicer et al, 2010), and the insertion sites are approximately 3.5 times greater in area (Harner et al, 1999). Structurally, the ligament is obliquely orientated within the knee and divided into collections of fascicles termed bundles (Arnoczky, 1983).…”

“…The tibial insertion of the ACL is considered the broadest part of the ligament and is described as either oval or triangular in shape (Bicer et al, 2010;Harner et al, 1999). Located in the anterior intercondylar fossa of the tibia and antero-lateral to the medial tibial spine, the area covered by the ACL is 7 -16 mm medial to lateral and 9 -19.5 mm anterior to posterior .…”
