Current Topics and Perspectives in Surgical Management of Hepatocellular Carcinoma

Abstract: Hepatocellular carcinoma is among the leading causes of cancer-related mortality. Due to the numerous surgical and non-surgical therapeutic options, the treatment strategy requires an optimal selection of patients based on tumor stage and liver functional reserve. A potentially curative surgical resection or liver transplantation is only recommended for patients with early stage disease. In this chapter, we overview the current topics and perspectives in the surgical management of hepatocellular carcinoma by d… Show more

“…Approximately 110,000 patients die of HCC every year in China, accounting for 45% of the death toll in the world ( 20 ). The therapeutic strategies for HCC mainly include chemotherapy, interventional therapy, surgery, liver transplantation and other comprehensive treatments ( 21 ). However, due to its high heterogeneity and high metastasis, it is still difficult to achieve good clinical effects ( 22 ).…”

“…Most HCC patients are diagnosed in the advanced stages, in which several treatment modalities are followed, including the tyrosine kinase inhibitor targeted therapy (sorafenib), systematic chemotherapy using cytotoxic agents (cisplatin and 5-fluorouracil), trans-arterial chemoembolisation, external irradiation, liver resection and orthotopic liver transplantation (OLT). Despite the variety of treatment protocols, only OLT and surgical resection are curative [ 9 , 10 ]. Sorafenib, a multiple-target tyrosine kinase inhibitor, is utilized as the first-line therapy for patients with advanced HCC stage.…”

“…All solitary tumors ≥2 cm were designated as stage A in the updated BCLC staging system (3). For stage A patients with preserved liver function and good performance status, surgical resection or liver transplantation is the recommended curative treatment (2)(3)(4)(5).…”
